Immune Response in Asymptomatic Paraproteinemia

This paper analyses impairment of the primary immune response of mice bearing the plasmacytoma TEPC-183. Healthy animals and mice bearing the reported non-immunosuppressive tumour MOPC-104E were used as controls. The defect was shown to affect both primary IgG and IgM responses to chicken cells (CRBC) and to be related to tumour size. However, the primary immune depression could be overcome either by increasing the antigen dose or by using Freund's complete adjuvant together with antigen. Secondary responses were also depressed. This depression was more pronounced if the animals was primed after, rather than before, tumour implantation. Further studies involved the measurement of primary immune responses of immunologically deprived syngeneic mice, after they had been reconstituted with cells from normal or tumour-bearing mice. Lymphocyte reconstitution experiments were carried out in mice which had been irradiated with 950R. Various lymphoid preparations from TEPC-183-bearing mice were unable to bring about such restoration. It is concluded that the impairment of the primary immune response of mice bearing the plasmacytoma TEPC-183 is due to a macrophage, rather than a lymphocyte, abnormality. However, none of these transfer studies suggested that positive suppression of primary immune responses was being mediated by cells from TEPC-183-bearing mice.

Several sensitive parameters of cellular immunity were studied in 20 patients with asymptomatic paraproteinaemia as judged after at least 3 years of follow‐up. In vitro lymphocyte transformation by the primary immunogen haemocyanin of Helix pomatia (HPH) following immunization, by PHA, PWM and previously encountered antigens was measured, as was skin sensitization by the primary immunogen dinitrochlorobenzene (DNCB). All 12 patients with low serum paraprotein levels—benign paraproteinaemia according to common criteria— had normal parameters of cellular immunity. However, 5 of 6 patients with high IgG and IgA serum paraprotein levels had a decreased response to HPH; 3 of these 5 also had a decreased PHA response. Two patients with high IgM serum paraprotein levels had normal lymphocyte reactivity. Lymphocyte reactivity by PWM and previously encountered antigens was normal in all patients. Autologous serum culture medium did not influence the results. DNCB skin sensitization was significantly lower in the group with high serum paraprotein levels than in the group with low levels. It is concluded, that in asymptomatic paraproteinaemia, patients with high serum paraprotein levels differ from those with low paraprotein levels in their defective cellular response to primary immunogens. The significance of these findings is discussed.

Immune responsiveness was investigated in a family comprising 12 first-and seconddegree relatives, one of whom had K-myelomatosis, one IgA-A paraproteinaemia, two Graves' disease and a further two thyroid antibodies without disease. Relatives by marriage served as controls. Parameters of immune capacity studied were the humoral and cellular immune response to haemocyanin of Helix pomatia (HPH), dinitrochlorobenzene (DNCB) skin reactivity and in vitro lymphocyte proliferation capacity to phytohaemagglutinin (PHA). Mean antibody titres to HPH were higher in the family than in the control group in all main Ig classes and IgG subclasses after primary and secondary immunization, and the difference was statistically significant for IgG, IgG, and IgG, titres. This could not have been predicted from the (normal) serum Ig levels in this family. In vitro lymphocyte proliferation capacity to HPH after primary and secondary immunization was also significantly increased. DNCB skin reactivity also tended to be high in the family, whereas PHA-induced lymphocyte proliferation was normal. These findings support the idea that myelomatosis clusters in families with immune dysregulation.
