Immediate versus Postponed Intervention for Infected Necrotizing Pancreatitis

Abstract: BACKGROUNDInfected necrotizing pancreatitis is a potentially lethal disease that is treated with the use of a step-up approach, with catheter drainage often delayed until the infected necrosis is encapsulated. Whether outcomes could be improved by earlier catheter drainage is unknown. METHODSWe conducted a multicenter, randomized superiority trial involving patients with infected necrotizing pancreatitis, in which we compared immediate drainage within 24 hours after randomization once infected necrosis was dia… Show more

“…In addition, only 18.37% of patients with ANP showed improvement after PCD intervention. This is lower than the 35–51% reported by other research institutes ( 3 , 7 , 9 , 16 , 17 ). It may be that our hospital is one of the largest acute pancreatitis diagnosis and treatment centers in northern China.…”

“…Although the comprehensive complication index score and mortality of the two groups were similar, it did not prove the superiority of early intervention in the treatment of patients with IPN. It is only suggested that early PCD intervention can be considered for IPN patients with rapid clinical deterioration ( 7 ). In this study, patients in the early group received PCD intervention after the failure of conservative antibiotic treatment.…”

“…Studies on delay in surgical source control were mostly conducted among patients with single disease entities such as necrotizing fasciitis, peptic ulcer perforation, cholangitis, infected pancreatic necrosis or urinary tract infections and mostly had small samples with heterogeneous disease severity [ 2 , 10 12 , 32 34 ]. Two recent large studies among patients with sepsis [ 13 ] and septic shock [ 14 ] did not find a significant increase in mortality by delayed source control.…”

“…Die Dringlichkeit der Therapie und somit auch der Druck zur Intervention richtet sich also nach dem klinischen Zustand. Liegt eine mutmaßlich infizierte Pankreasnekrose vor, so sollte zunächst mit einem Breitbandantibiotikum behandelt werden und eine Intervention im Step-up Approach nur bei fehlender klinischer Besserung erfolgen [15]. Vor jeder geplanten Intervention ist eine Schnittbildgebung mit Kontrastmittel obligat.…”
