ResumoNovas técnicas reabilitadoras protéticas têm sido desenvolvidas no intuito de diminuir o tempo de tratamento, assim como o custo e o trauma cirúrgico dos pacientes. Além disso, minimizam agravos psicológicos, principalmente em regiões com envolvimento estético. A Instalação imediata de implantes já e uma realidade e estudos clínicos têm demonstrado que a taxa de sucesso dos implantes imediatos é similar a dos implantes instalados após o reparo ósseo. O objetivo deste trabalho é apresentar um caso clínico de instalação de um implante em substituição imediata após a exodontia atraumática, através do uso do extrator radicular Neodent. Implantes imediatos em carga imediata, quando corretamente indicados, são uma alternativa rápida e segura para a reabilitação de dentes condenados, pois permitem a preservação da arquitetura óssea, além de proporcionar ao paciente um menor número de cirurgias e consequentemente menor morbidade, além de proporcionar ao paciente conforto psicológico e satisfação estética e funcional imediata.Palavras-chave: Implantes dentários; Prótese dentária fixada por implante; Procedimentos cirúrgicos minimamente invasivos.

Aim: The aim of this study was to evaluate the success rates of implants placed in alveolar sockets immediately after the extraction of teeth or with a standard approach in normal alveolar bone in periodontally compromised patients. Material and Methods: In this study, patients who had implant placement both into healed bone after previous extraction and into the alveolar sockets in the same surgical session after extraction of periodontally hopeless teeth that had inadequate attachment and bone loss who could not be treated were selected for inclusion. Implant success, complications, and failures were recorded during follow-up. Results: Twelve patients (four female and eight male) ranging in age from 27 to 62 years (mean 48.6 years) were studied; the mean duration of follow-up was 27 months (range 18-36 months).. A total of 152 implants were placed (89 in the maxilla, 63 in the mandible). Of these, 83 (54.6%) were placed immediately into alveolar sockets following extraction and 69 (45.4%) into normal alveolar bone. The success rate for all immediately placed implants was 97.6%, and the success rate for all implants placed as non-immediate was 98.6 %. Conclusion:The study findings show that both immediate implant placement and non-immediate implant placement in periodontally compromised patients receiving treatment of implant-supported fixed full or partial prostheses are safe and preferable treatment modalities.

ABSTRACT:Bisphosphonates are anti-resorptive drugs commonly prescribed for management of osteoporosis. In spite of its clinical efficacy, a known adverse effect of these drugs is bisphosphonate induced osteonecrosis of the jaw (BRONJ). Dentists usually try to avoid invasive surgical procedures among patients consuming this category of drugs. This case report describes successful placement of multiple implant supported prosthesis in an elderly osteoporotic female with four years history of oral bisphosphonate therapy. Besides serum vitamin D and calcium level, the standard investigation to assess bone turn over in such patient is carboxyl-terminal crosslinking telopeptide of type I collagen (CTX). Since no local laboratory offered the CTX test; an alternative investigation N-terminal crosslinking telopeptide of type I collagen (NTX) was offered. Careful treatment planning and gentle surgical approach made it possible to immediately load the implant supported prosthesis in her both arches. This case report reiterates the fact that successful implant supported prosthesis can be placed in patients on long term oral bisphosphonates, provided comprehensive treatment planning including relevant investigations are done and the surgical trauma is kept to a minimum.
