Immediate Management of Posterior Urethral Disruptions Due to Pelvic Fracture: Therapeutic Alternatives

Abstract: As described, urethral alignment was not beneficial for avoiding urethral obliteration. Therefore we recommend suprapublic cystostomy as the only form of initial treatment in these cases. Urinary incontinence seems more likely related to associated bladder neck rupture and the severity of pelvic fracture rather than to initial treatment or delayed urethral repair. Consequently, when associated bladder neck injury is present, we advocate immediate surgical repair.

“…Boone et al [14] noted that three distinct types of urethral injury (supraprostatic, transprostatic and prostatomembranous) are present in this population. Moreover, because of a small prostate, violent pelvic injuries in childhood may be responsible for complete rupture of the membranous urethra as well as urethral disruption at the bladder neck [24,25]. In addition, Glassberg et al described in children extension of the urethral disruption injury at the prostatomembranous junction into the proximal bulbar urethra [26].…”

“…Although these complications seem to be related to the severity of the primary injury of the pelvic trauma and associated bladder neck rupture [24], prompt surgical repair cannot be recommended due to the often critical general state and associated injuries of the child. Immediate open repair may remove the tamponade effect caused by the pelvic hematoma and thus carries the potential for hemorrhage.…”

“…[42] However, some series in the past have reported a larger percentage of use of the transpubic technique ranging from 35% to 75%. [194344] Transpubic approach may be needed in children with defects more than 3 cm long. [43]…”
