Immediate changes in the mandibular dentition after maxillary molar distalization using headgear

Abstract: The purpose of this study was to investigate immediate changes in the mandibular dentition after maxillary molar distalization using headgear in non-growing patients. Sixteen patients (mean age, 18.9 ± 2.0 years) with Class II molar relationship and crowding were included in the present study. To correct the molar relationship, headgear was used for maxillary molar distalization. Cone-beam computed tomography-generated half-cephalograms (CG Cephs) and dental casts were used to evaluate dental changes for each … Show more

“…The lengthening and widening of the maxillary dental arch as a consequence of CH treatment can cause spontaneous space correction in the mandibular dental arch. This phenomenon is followed by widening of the mandibular arch (4)(5)(6)(7)9). A similar spontaneous effect on the mandibular arch has also been reported after Haas-type maxillary expansion (10,11).…”

“…In the HG therapy, expansion of the inner facebow arch can be applied up to 10 mm in order to broaden the upper dental arch while achieving at the same time as distal movement of the first molars and restriction of forward displacement of the maxilla ( 15 , 25 , 40 ). Widening of the maxillary molars has been found to lead to spontaneous buccal tipping of the mandibular molars, supposedly due to occlusal factors, and eventually to mild changes in the mandibular dental arch width ( 15 , 20 , 22 , 27 , 41 , 52 ). This study is in agreement with the previous studies.…”

“…Moreover, additional advantages of distalising the dentitions rather than premolar extractions include the preservation of a complete dentition, the ease of root position control, the convenience of controlling the amount of anterior tooth retraction, as well as good patient acceptance. 17 The Invisalign system has become increasingly popular, especially for adult patients, [18][19][20] since it was first introduced in 1997, due to the advantages of aesthetics and comfort in comparison with fixed appliances. 21 The clinical efficiency of clear aligner treatment, including molar distalisation, has been confirmed and it has been further suggested that tooth movement, such as incisor torque, premolar derotation and molar distalisation, can be effectively performed using the Invisalign system.…”
