Immediate and dual response to nasal challenge with Dermatophagoides pteronyssinus in local allergic rhinitis

Abstract: The results demonstrated the existence of immediate and dual responses to a NAPT with DP in LAR patients, with the local presence of sIgE and mast cell/eosinophil activation.

“…Based on the obtained results, D. pteronyssinus was the predominant allergen that caused AR or LAR in the analyzed groups, a result in agreement with previous data, which showed that D. pteronyssinus may have great importance in the induction of LAR among various age groups [11,12]. The course of nasal symptoms, regardless of severity, in this type of allergy can mimic chronic non-AR; therefore, in a significant group of patients, the true diagnosis of LAR was significantly delayed.…”

“…In recent years, local allergic rhinitis (LAR) has been defined as a new phenotype of allergic rhinitis characterized by nasal symptoms of allergic rhinitis in the absence of sensitivity in a skin prick test (SPT) or serum-specific IgE, a positive response to a specific nasal provocation test (NPT) and the presence of a Th2 immune inflammatory response in the nasal mucosa with local production of specific IgE [1,2,3,4,5]. Powe et al [6] enounced ‘entopy' by describing the activation of a Th2 immune response in the nasal mucosa of nonallergic rhinitis patients.…”

“…Specific IgE identified using the enzyme allergosorbent test (EAST) can be detected in nasal secretions but not in the serum of these patients [1]. This type of allergy is limited to the nasal mucosa and therefore referred to as local allergic rhinitis [2,3], entopy [4] or allergic rhinitis with a negative skin test [5]. Clinical symptoms, nasal secretions and the profile of cytokine production of local allergic rhinitis are similar to classic allergic rhinitis [6,7].…”
