L entigo maligna (LM) is an in situ lesion with a 2% to 50% risk of progression to LM melanoma. 1 Currently, surgery or radiotherapy is usually recommended as the primary treatment for LM. In the literature, the recurrence rates reported for radiotherapy range from 0% to 19%, with a mean recurrence rate of approximately 7%; in addition, radiotherapy carries the risk of causing chronic radiodermatitis or radiation-induced malignant neoplasm. 2 A margincontrolled excision using "slow Mohs" (rush permanent sections) and Mohs micrographic surgery has the lowest recurrence rate, perhaps as low as 3%. 3Methods. Thirty-two patients with 34 histologically confirmed facial LM lesions were enrolled in an open-label trial of imiquimod, 5%, cream (Aldara; 3M Pharma, Rueschlikon, Switzerland). “…A meta analysis published in 2006 identified a total of 11 case reports and 4 open-label studies, including a total of 67 lentigo maligna patients treated with topical imiquimod. Most patients experienced complete clinical remissions (Rajpar and Marsden, 2006). This review will briefly cover some important aspects of skin structure and function before touching upon fundamental principles of neoplastic cell growth in the skin and some of the important molecular pathways involved. After presenting evidence for a role of the immune system in shaping the development of skin cancer, concepts for tumor immunotherapy with TLR-agonists are introduced from a historical point of view. Subsequently, the use of synthetic DNA, synthetic RNA and synthetic small immunostimulatory molecules for immunotherapy of early forms of epithelial carcinoma (actinic keratoses) and melanoma (lentigo maligna), as well as for advanced metastatic melanoma, is comprehensively presented. Finally, current developments and future prospects for immunotherapy of occult or unresectable melanoma metastastases, the most important clinical problem today, are discussed.

Malignant melanoma represents a particular challenge for dermatologists and oncologists because of its high and increasing incidence and the poor prognosis of patients with thick primary tumors (T3, T4). In advanced stages of melanoma, cutaneous and subcutaneous metastases have a special significance, as they markedly affect the patient and may lead to a limitation in quality of life. While topical therapy is possible, there are only limited clinical studies. The location, number, size and distribution of skin metastases, involvement of internal organs, age and general condition should be considered in assessing therapeutic options. Especially with solitary, easily accessible metastases, surgical excision represents the therapy of choice. Ablation using CO(2) laser is an alternative. With extensive metastases in just one extremity, isolated limb perfusion (ILP) with melphalan is an option, while multiple, smaller metastases can be irradiated. Further, several chemotherapeutic agents and immune modulators can be used topically, peri- and intralesionally.

Abstract: Although imiquimod clearly has an effect on LM, this analysis of available studies has helped to identify concerns about its use. Without controlled evidence and prolonged follow up, the use of imiquimod for LM must still be considered experimental.
