The aim of this study was to compare the diagnostic performance of clinical visual examination (ICDAS II), digital periapical radiography (PR), near infrared light transillumination (NIR-LT), and laser fluorescence (LF) to microcomputed tomography (Micro-CT) which is the reference standard for the detection of non-cavitated occlusal enamel and dentin caries in third molar teeth. Potential participants were consecutively recruited. In this prospective study, 101 third molars of 101 patients were examined; the molars had non-cavitated occlusal caries which required extraction. ICDAS II, PR, NIR-LT and LF examinations were carried out by two blinded examiners. Reference standard was determined by micro-CT imaging seven days after extraction. Accuracy rate, sensitivity, specificity, predictive values and areas under receiver operating characteristic (ROC) curves (Az) were statistically analyzed. Nonparametric variables were subjected to the Kruskal–Wallis Test. Significance level was set as p < 0.05. NIR-LT had the highest sensitivity (99.67–99.67%) and accuracy (78.22–77.23%) for the determination of occlusal enamel caries according to examiners 1 and 2, respectively. LF method had the highest sensitivity (70.83–54.17%) and accuracy (66.34–59.41%) for determining occlusal dentin caries according to examiners 1 and 2, respectively. The ROC curve (Az) value ranged between 0.524 and 0.726 for the different methods tested. Most effective methods for the diagnosis of occlusal enamel and dentin caries were determined to be NIR-LT and LF pen methods, respectively. The present prospective clinical study showed that NIR-LT and LF-Pen were a reliable modality for the detection of occlusal enamel and dentin caries without ionizing radiation.

Enamel Caries Detection and Diagnosis: An Analysis of Systematic Reviews

Detection and diagnosis of caries—typically undertaken through a visual-tactile examination, often with supporting radiographic investigations—is commonly regarded as being broadly effective at detecting caries that has progressed into dentine and reached a threshold where restoration is necessary. With earlier detection comes an opportunity to stabilize disease or even remineralize the tooth surface, maximizing retention of tooth tissue and preventing a lifelong cycle of restoration. We undertook a formal comparative analysis of the diagnostic accuracy of different technologies to detect and inform the diagnosis of early caries using published Cochrane systematic reviews. Forming the basis of our comparative analysis were 5 Cochrane diagnostic test accuracy systematic reviews evaluating fluorescence, visual or visual-tactile classification systems, imaging, transillumination and optical coherence tomography, and electrical conductance or impedance technologies. Acceptable reference standards included histology, operative exploration, or enhanced visual assessment (with or without tooth separation) as appropriate. We conducted 2 analyses based on study design: a fully within-study, within-person analysis and a network meta-analysis based on direct and indirect comparisons. Nineteen studies provided data for the fully within-person analysis and 64 studies for the network meta-analysis. Of the 5 technologies evaluated, the greatest pairwise differences were observed in summary sensitivity points for imaging and all other technologies, but summary specificity points were broadly similar. For both analyses, the wide 95% prediction intervals indicated the uncertainty of future diagnostic accuracy across all technologies. The certainty of evidence was low, downgraded for study limitations, inconsistency, and indirectness. Summary estimates of diagnostic accuracy for most technologies indicate that the degree of certitude with which a decision is made regarding the presence or absence of disease may be enhanced with the use of such devices. However, given the broad prediction intervals, it is challenging to predict their accuracy in any future “real world” context.

In the practice of a pediatrician, among all detected infectious diseases, acute respiratory infections (ARI) continue to lead. The causes of respiratory infections can be varied. Great importance is attached to the normal microflora, as one of the main regulatory factors that can ensure the adaptation of children to different living conditions, and is responsible for maintaining homeostasis and maturation of the immune system. It has now been established that a healthy oral microbiome is the key to somatic health, and one of the important components of anti-infective protection. Therefore, one of the promising areas for the prevention of respiratory infections is the use of oral probiotics. The article provides an overview of findings from the studies on the efficacy and safety of the probiotic strain Streptococcus salivarius K12 showing inhibitory activities against the main respiratory pathogens – Streptococcus pyogenes, Streptococcus pneumoniae, Haemophilus influenzae, Moraxella catarrhalis. S. salivarius K12 produces the bacteriocin-like inhibitory substance BLIS K12 (Bacteriocin-Like Inhibitory Substance), which plays an important role in maintaining the natural flora of the upper respiratory tract. It has been proven that the use of S. salivarius K12 results in a reduction of the level of pathogen colonization of the oropharyngeal mucosa, a rapid decline of the clinical symptoms of acute and chronic diseases of the ENT organs, and the prevention of acute respiratory diseases. The reason for this is that the intake of S. salivarius K12 increases the amount of interferon γ in saliva, stimulating the adaptive immunity against many viral infections. S. salivarius K12 is included in the clinical guideline on acute tonsillopharyngitis for the prevention of relapses, as well as the recovery of the normal biota of the oropharyngeal mucosa after disease.
