Imaging modalities in focal therapy

Abstract: Purpose of review Focal therapy for prostate cancer is emerging as a management option between active surveillance and radical treatments. In this article we present two of the most important imaging modalities in focal therapy, multiparametric MRI and Ultrasonography. We review recent advances within these two platforms. Recent findings State-of-the-art imaging in all phases of focal therapy is essential for treatment safety. In patient selection, treatment guidance and follow-up, different aspects of imagi… Show more

“…In CETRUS recordings, the entire prostate is represented by groups of data points; moreover, current studies often focus on the PZ hyperperfusion region rather than the whole prostate and do not conduct tissue analysis of other normal tissue181920. Here, we show that incorporating normal tissue TICs could significantly improve the diagnostic accuracy of nodule TICs for PCa detection (Fig.…”

“…(PSA is not helpful for monitoring FT outcome 42 ). Histologic post FT assessment depends on either random transrectal or transperineal approach 16,43 . Transrectal option is prone to substantial sampling error and a high rate of false negative results.…”

“…Transperineal mapping option requires repeat general anesthesia 44 . mpMRI offers the promise to guide post-FT biopsy and overcome these limitations 42,43,45,46 . However there are obvious concerns related to tissue displacement 47 .…”

“…Furthermore, it can detect SVI and ECE as well as pelvic LNI at satisfactory levels in the follow-up of patients who are deemed as clinically insignificant. Its sensitivity is reported as >80% and specificity is reported as >90% in the latest series (27)(28)(29)(30). However, the results of mp-MRI are not yet included within the scope of active surveillance criteria or decision algorithm.…”
