Imaging features of focal intrahepatic extramedullary haematopoiesis.

Abstract: The imaging findings of focal intrahepatic extramedullary haematopoiesis (EMH) in a 51-year-old woman with beta-thalassaemia intermedia are described with particular reference to MRI and CT. Bone marrow colloid scintigraphy was unhelpful in confirming the diagnosis, which was made from fine needle aspiration. This is the first description of stellate scars occurring in an EMH lesion. A review of the radiological appearances of this rare condition is presented.

“…1,2 In general, the reported characteristics of EMH on ultrasound, computed tomography, and MRI are quite variable, and even if clinical suspicion is high, these techniques are at best suggestive. 11,12 A number of radiotracers have been used to demonstrate EMH based on the various physiologic activities of the marrow. Perhaps the best known of these radiotracers are the colloids, such as Technetium-99m-colloid (99 mTc), which are taken up by macrophages.…”

“…Radionuclide bone marrow scanning using Fe citrate are helpful in establishing the diagnosis by demonstrating activity in the mass [3,4,10] . However, caution should be exercised in the use of these modalities as the sole method of investigation because of the occasionally reported failure of these investigations to detect [10] and differentiate EMH from lymphoma [11] .…”

“…However, caution should be exercised in the use of these modalities as the sole method of investigation because of the occasionally reported failure of these investigations to detect [10] and differentiate EMH from lymphoma [11] . Therefore, the exclusion of malignancy obviates the need for tissue biopsy, preferably by a wide-bore (18 G) needle or Tru-Cut biopsy, as in our case.…”

“…Microscopically, there is diffuse infiltration of hematopoietic cells, altough there may be tumor-like masses of hematopoietic tissue with a variable degree of fibrosis [8,9]. Only 10-12 cases of focal intrahepatic EMH have been reported [10][11][12]. Few cases of nodular EMH diagnosed by FNA have been reported in the literature [13].…”
