Imaging Assessment of Endothelial Function: An Index of Cardiovascular Health

Abstract: Endothelial dysfunction is a key early mechanism in a variety of cardiovascular diseases and can be observed in larger conduit arteries as well as smaller resistance vessels (microvascular dysfunction). The presence of endothelial dysfunction is a strong prognosticator for cardiovascular events and mortality, and assessment of endothelial function can aid in selecting therapies and testing their response. While the gold standard method of measuring coronary endothelial function remains invasive angiography, se… Show more

“…Many of these markers have been associated with all-cause mortality and some of these with CV mortality in PWH. 6,[20][21][22][23] Therefore, the chronic immune activation and inflammation present in PWH despite stable ART likely interacts with the immune and inflammatory processes of vascular repair and atherosclerosis. Of importance, our findings highlight that these processes are operative in the setting of coronary endothelial dysfunction (an early stage of coronary atherosclerosis), which has been investigated far less than systemic endothelial function using FMD among PWH.…”

“…Flow-mediated dilation (FMD) has frequently been used in research, though is considered an assessment of peripheral endothelial function, as opposed to coronary function [27]. Cardiac MRI and cardiac PET have also been used in the evaluation of coronary endothelial function (CEF) when combined with isometric handgrip exercise (IHE) or cold pressor challenge, respectively [28].…”
