Image-guided high-dose-rate brachytherapy in inoperable endometrial cancer

Abstract: Inoperable endometrial cancer may be treated with curative aim using radical radiotherapy alone. The radiation techniques are external beam radiotherapy (EBRT) alone, EBRT plus brachytherapy and brachytherapy alone. Recently, high-dose-rate brachytherapy has been used instead of low-dose-rate brachytherapy. Image-guided brachytherapy enables sufficient coverage of tumour and reduction of dose to the organs at risk, thus increasing the therapeutic ratio of treatment. Local control rates with three-dimensional b… Show more

“…Twenty percent of deaths from cancer are due to gynecological (GYN) malignancies [ 1 ]. 3D image guided brachytherapy (IGBT) in combination with external beam radiotherapy (EBRT) is a favorite treatment option for all stages of cervical cancer, especially for inoperable patients [ 2 , 3 , 4 , 5 , 6 ]. To achieve sufficiently high-dose levels to the tumor, organs at risk (OARs) should be spared to reduce radiation related morbidity.…”

“…In the recent years, three-dimensional image-guided brachytherapy[ 7 8 9 10 ] is slowly replacing the conventional X ray-based, point A-based dose reporting in gynecological brachytherapy. Groupe Européen de Curiethérapie– European Society for Radiotherapy and Oncology has published guidelines on target volume definitions and DVH parameters.…”

“…The American Brachytherapy Society (ABS) developed consensus guidelines for medically inoperable cases [ 2 ]. Literature on surgically unresectable endometrial cancer is sparse and heterogeneous, making it challenging to apply to modern techniques [ 3 ]. Although there are limited data on the outcomes of localized unresectable stage III patients, ten-year crude local failure rates of 65% (32/49) to 67% (4/6) have been reported [ 4 - 5 ].…”
