Aims: To test the validity of diagnostics incorporating digital image analysis (DIA) for human epidermal growth factor 2 (HER2) immunohistochemistry (IHC) in gastro-oesophageal adenocarcinomas, as an alternative to current standard diagnostics using manual scoring. Methods and results: We included 319 consecutive gastro-oesophageal adenocarcinomas (232 biopsies and 87 surgical specimens). DIA was applied to determine HER2 IHC classification, using both standard breast cancer (BC) and modified gastro-oesophageal cancer (GEC) cut-offs. Consensus manual scores were established by four independent observers. Chromogenic in-situ hybridization (CISH) was performed on all 2+ cases by manual scoring, DIA or both. HER2 status was considered positive in 3+ and CISHpositive 2+ cases. Overall agreement between DIA and consensus manual scores was 76.5% (weighted j = 0.66, BC cut-offs) and 85.6% (weighted j = 0.80, GEC cut-offs). Agreement was similar for biopsies and surgical specimens. All disagreement occurred in the manual IHC equivocal cases. DIA resulted in a reduction of 2+ cases: 75.8% with BC cut-offs and 46.5% with GEC cut-offs. HER2 status was positive in 48 cases (15%) with standard diagnostics and DIA using GEC cut-offs, and 46 cases (14.4%) using BC cut-offs (all with CISH in 2+ cases). Considering standard diagnostics as a reference, DIA showed 93.8% sensitivity and 99.6% specificity (BC cut-offs) or 97.9% sensitivity and 99.6% specificity (GEC cut-offs). Conclusions: DIA is a reliable and feasible alternative to manual HER2 IHC scoring in gastro-oesophageal adenocarcinoma, both in biopsies and surgical specimens, leading to a reduction of 2+ cases for which subsequent ISH testing is required.

IntroductionThis study was designed to investigate the combined influence of ATM and Ki67 on clinical outcome in early stage hormone receptor positive breast cancer (ES-HPBC), particularly in patients with smaller tumors (< 4 cm) and fewer than four positive lymph nodes.Methods532 formalin-fixed paraffin-embedded specimens of resected primary breast tumors were used to construct a tissue microarray. Samples from 297 patients were suitable for final statistical analysis. We detected ATM and Ki67 proteins using fluorescence and brightfield immunohistochemistry respectively, and quantified their expression with digital image analysis. Data on expression levels were subsequently correlated with clinical outcome.ResultsRemarkably, ATM expression was useful to stratify the low Ki67 group into subgroups with better or poorer prognosis. Specifically, in the low Ki67 subgroup defined as having smaller tumors and no positive nodes, patients with high ATM expression showed better outcome than those with low ATM, with estimated survival rates of 96% and 89% respectively at 15 years follow up (p = 0.04). Similarly, low-Ki67 patients with smaller tumors, 1-3 positive nodes and high ATM also had significantly better outcomes than their low ATM counterparts, with estimated survival rates of 88% and 46% respectively (p = 0.03) at 15 years follow up. Multivariable analysis indicated that the combination of high ATM and low Ki67 is prognostic of improved survival, independent of tumor size, grade, and lymph node status (p = 0.02).ConclusionsThese data suggest that the prognostic value of Ki67 can be improved by analyzing ATM expression in ES-HPBC.

Background: Alpha-fetoprotein-positive gastric cancer (AFPGC) is a subtype of gastric cancer that is rare in clinical practice and extremely malignant. Malignant tumors are often associated with hemorrhage, thrombosis, and even disseminated intravascular coagulation (DIC). The D-dimer test is used as a sensitive index in the diagnosis of DIC and fresh thrombosis in malignant tumors. Therefore, this study aims to investigate the relationship between D-dimer values and the clinical characteristics and prognosis of patients with serum AFPGC (AFP ⩾ 15 μg/L) patients. Methods: Overall, 120 healthy subjects and 120 AFP-negative gastric cancer (AFP < 15μg/L) patients from May 2017 to July 2018 at the Shanxi Cancer Hospital served as the control group in this retrospective cohort study. Additionally, 120 patients with pretreatment advanced serum AFP were chosen to analyze clinicopathologic features and factors that affect prognosis. The predictor was the D-dimer, and the outcome variable was overall survival (OS). Other variables included age, sex, tumor site, T-stage, distant metastasis, and preoperative serum tumor biomarkers. Differences in OS rate were analyzed by GraphPad Prism 9.2.0.332. The Cox regression model was used for univariate and multivariate analysis. Results: In comparison to AFP-negative gastric cancer, we discovered that D-dimer had a meaningfully higher presentation in patients with AFPGC ( P < .001). Based on D-dimer median levels, the AFPGC patients were divided into two groups, including 39 patients with low D-dimer (<1000 ng/mL) and 81 patients with high D-dimer (⩾1000 ng/mL). The variables, including T-stage, distant metastasis, and expression of HER2, were associated with the value of D-dimer. The D-dimer levels were weakly related to the levels of tumor markers. The differences in AFPGC patients, with an OS rate of 30.76% for patients with low D-dimer (<1000) and 12.30% with high D-dimer (⩾1000; P = .0027), were statistically significant. Cox multivariate analysis of various parameters indicated that T-stage, distant metastasis, vascular embolism, level of D-dimer, and tumor biomarkers of AFP were independent risk factors for survival. Conclusion: Serum D-dimer levels may be a valuable indicator for predicting AFPGC metastasis and progression.
