Ileus Management International (IMAGINE): protocol for a multicentre, observational study of ileus after colorectal surgery

Abstract: Aim The management of postoperative ileus following colorectal surgery remains controversial. It is the commonest complication after elective colorectal resection and is associated with an increased incidence of postoperative adverse events. The prevention and management of postoperative ileus remains unstandardized. This study aims to describe an international profile of gastrointestinal recovery after colorectal surgery and will assess the role of non-steroidal anti-inflammatory drugs, when used as postopera… Show more

“…Factors associated with increased likelihood of achieving > 95% data completeness were lower numbers of patients to be followed-up, and central storage of patient hospital identifiers. As TRCs have now been set up across Europe [20, 21], validating this methodology will have broad international benefits.…”

“…Ileus is relatively common after colorectal surgery and may cause several issues. Consequently, probiotics can possibly decrease postoperative ileus by controlling bacterial metabolites and pH levels, as suggested by the results of the trial in this review [35][36][37].…”

“…Phagocytes present throughout the gut are triggered, resulting in a migration of leucocytes to the muscularis externa (13). The release of nitric oxide, cyclooxygenase-2 (COX-2) metabolites, and prostaglandins by these phagocytes prevents peristalsis by inhibiting smooth muscle contractility directly which leads to POI (14,39,41).…”

“…There is evidence in the literature that overall compliance of more than 70% to the ERAS programme as well as minimally invasive surgery helped prevent POI and therefore significantly minimised POI-associated medical complications (19,41). Treatment should include components of ERAS aimed at preventing the inflammatory response early on in the inflammatory cascade activated by intestinal handling during surgery (14,19).…”
