Different reasons can contribute to classifying dental prosthesis wearers as high-risk individuals in relation to dental biofilm accumulation. These include a past history of oral disease, age and additional retentive areas. Other common complaints include inflammation and halitosis. Moreover, prosthesis replacement and prosthetic pillar loss are generally associated with caries and periodontal disease recurrence. Therefore, the present study undertook to make a critical review of the literature, aiming at discussing the main aspects related to chemical agent prescriptions for dental prosthesis wearers. Most of the articles were selected based on relevance, methods and availability in regard to the specific subject under investigation, without considering publication year limitations. Different types of prostheses and their impact on teeth and other oral tissues were reported. It was demonstrated that there is greater biofilm buildup and increased inflammatory levels in the presence of different types of prostheses, suggesting that additional measures are required both on population-wide and individual levels in order to control these factors. Mechanical control consists of a combination of manual or electric toothbrush and toothpaste, as well as specific devices for interdental cleaning. Although many chemical agents exhibit antimicrobial benefits when used for prosthesis disinfection, only a few agents can be used safely without causing damage. Regarding the selection of antiseptics by the overall population, chlorhexidine is the most indicated in the short term and in sporadic cases. The most indicated adjuncts to overcome the deficiencies and limitations of daily mechanical biofilm control are products containing essential oils as active ingredients.

Introduction: The aim of this study was to evaluate the levels of C-reactive protein (CRP) and interleukin-6 (IL-6) in the gingival crevicular fluid (GCF) among stroke survivors in relation to the oral hygiene status and stroke characteristics. Methods: A multi-centre cross-sectional study was conducted among hospitalised stroke survivors. Socio-demographic data were collected. Oral clinical assessment (dental plaque scores) and functional dependency levels scores were carried out. Gingival Crevicular Fluids were collected, using absorbent papers and analysed using ELISA kit. Descriptive statistic and correlation analyses were performed using proportion and Spearmen correlation coefficient test. Results: A total of 53 patients were recruited from five public hospitals. There was a significant correlation between CRP and IL-6 levels of GCF (P=0.021, r=0.21). A significant correlation was also observed between CRP levels in GCF with dental plaque scores and functional dependency levels. There was no statistically significant correlation found between IL-6 levels in GCF with dental plaque scores and functional dependency levels. Dental plaque scores were associated with high levels of CRP in GCF (P=0.014), and swallowing problem was associated with high levels of IL-6 in GCF (P=0.002). Conclusions: IL-6 levels in GCF were correlated with the levels of CRP in GCF. High CRP and IL-6 levels in GCF were significantly associated with dental plaque scores and swallowing condition (presence of dysphagia), respectively. Thus, this pilot study suggests that CRP level in the oral cavity respond to the oral health conditions and may not be a predictor factor of stroke outcomes. Further studies are warranted to compare the level of inflammatory biomarkers from the oral cavity and serum in relation to the stroke conditions and outcomes.

Potential Impact of Prosthetic Biomaterials on the Periodontium: A Comprehensive Review

The success of a prosthetic treatment is closely related to the periodontal health of the individual. The aim of this article was to review and present the importance of prosthetic restorative materials on the condition of the periodontium, the changes that occur in the composition of the subgingival microbiota and the levels of inflammatory markers in gingival crevicular fluid. Articles on the influence of different prosthetic restorative materials on subgingival microbiota and proinflammatory cytokines were searched for using the keywords “prosthetic biomaterials”, “fixed prosthesis”, “periodontal health”, “subgingival microbiota”, “periodontal biomarkers” and “gingival crevicular fluid” in PubMed/Medline, Science Direct, Scopus and Google Scholar. The type of material used for prosthesis fabrication together with poor marginal and internal fit can result in changes in the composition of the subgingival microbiota, as well as increased accumulation and retention of dentobacterial plaque, thus favoring the development of periodontal disease and prosthetic treatment failure. Biological markers have helped to understand the inflammatory response of different prosthetic materials on periodontal tissues with the main purpose of improving their clinical application in patients who need them. Metal-free ceramic prostheses induce a lower inflammatory response regardless of the fabrication method; however, the use of CAD/CAM systems is recommended for their fabrication. In addition, it is presumed that metal-ceramic prostheses cause changes in the composition of the subgingival microbiota producing a more dysbiotic biofilm with a higher prevalence of periodontopathogenic bacteria, which may further favor periodontal deterioration.
