INTRODUÇÃOA HIPERTENSÃO ARTERIAL, uma entidade clínica multifatorial, é conceituada como síndrome caracterizada pela presença de níveis tensionais elevados, associados a alterações metabólicas e hormonais e a fenô-menos tróficos (hipertrofias cardíaca e vascular). A prevalência da hipertensão arterial é elevada, estimando-se que cerca de 15% a 20% da população brasileira adulta possa ser rotulada como hipertensa. Embora predomine na fase adulta, sua prevalência em crianças e adolescentes não é desprezível.Considerada um dos principais fatores de risco de morbidade e mortalidade cardiovasculares, seu alto custo social é responsável por cerca de 40% dos casos de aposentadoria precoce e de absenteísmo no trabalho em nosso meio.Devido à magnitude do problema, tem sido constante a preocupação mundial em ampliar e aperfeiçoar os métodos para diagnóstico e tratamento da hipertensão arterial.Assim, diante das novas aquisições científicas e tecnológicas, tornou-se necessária a revisão dos conceitos e indicações clínico terapêuticas constantes do documento do II Consenso Brasileiro de Hipertensão Arterial, produzido em 1994. Este novo documento contempla também a moderna abordagem multiprofissional do paciente hipertenso e a importância da implementação de estratégias visando à prevenção primária da hipertensão arterial.O documento do Consenso e um guia prático, que visa a ajudar o médico e o demais profissionais de saúde no diagnóstico e tratamento da hipertensão arterial. Desse modo, as recomendações contidas neste documento deverão ser adaptadas e implementadas de acordo com a necessidade individual de cada paciente e o julgamento clínico do profissional de saúde.
CAPÍTULO 1Diagnóstico e Classificação O diagnóstico da hipertensão arterial é basicamente estabelecido pelo encontro de níveis tensionais permanentemente elevados acima dos limites de normalidade, quando a pressão arterial é determinada por meio de méto-dos e condições apropriados. Portanto, a medida da pressão arterial é o elemento-chave para o estabelecimento do diagnóstico da hipertensão arterial.
Medida Indireta da Pressão ArterialA medida da pressão arterial, pela sua importância, deve ser estimulada e realizada, em toda avaliação de saúde, por médicos de todas as especialidades e demais profissionais da área de saúde.O esfigmomanômetro de coluna de mercúrio é o ideal para essas medidas. Os aparelhos do tipo aneróide, quando usados, devem ser periodicamente testados e devidamente calibrados. A medida da pressão arterial deve ser realizada na posição sentada, de acordo com o procedimento descrito a seguir:
