Purpose: Complete surgical resection is the main determining factor in the survival of advanced gastric cancer (AGC) patients, but resection should be avoided in metastatic disease. Peritoneum is a common site of metastasis and preoperative imaging techniques still fail to detect peritoneal metastasis (PM). Thus, the aim of this study was to evaluate the role of Staging Laparoscopy (SL) in the staging of AGC patients in a Western tertiary cancer center. Methods: We reviewed 130 patients with gastric adenocarcinoma submitted to SL from 2009 to 2020 from a prospective database. Clinicopathological characteristics were analyzed to identify factors associated with the presence of PM. We also evaluated the accuracy and strength of agreement between computed tomography (CT) and SL in detecting PM, and the change in treatment strategy after SL. Results: Among the 130 patients, PM was identified in 66 patients (50.76%) - P1 group. The sensitivity, specificity and accuracy of CT in detecting PM were 51,5%, 87,5% and 69.2%, respectively. According to the Kappa coefficient, concordance between SL and CT was 38.8%. In multivariate analysis, ascites (p=0.001) and suspected PM on CT scan (p=0.007) were statistically correlated with the P1 group. In 40 patients (30.8%), staging and treatment plans changed after SL (32 patients avoided unnecessary laparotomy and 8 patients who were previously considered stage IVb by CT scan were referred for surgical treatment). Conclusions: Even with current advances in imaging techniques, SL demonstrated an important role in the diagnosis of PM and remains valuable for determining the correct therapeutic strategy.

Background: Southern Brazil has one of the highest incidences of esophageal squamous cell carcinoma in the world. Transthoracic esophagectomy allows more complete abdominal and thoracic lymphadenectomy than transhiatal. However, this one is associated with less morbidity. Aim: To analyze the outcomes and prognostic factors of squamous esophageal cancer treated with transhiatal procedure. Methods: All patients selected for transhiatal approach were included as a potentially curative treatment and overall survival, operative time, lymph node analysis and use of neoadjuvant therapy were analyzed. Results: A total of 96 patients were evaluated. The overall 5-year survival was 41.2%. Multivariate analysis showed that operative time and presence of positive lymph nodes were both associated with a worse outcome, while neoadjuvant therapy was associated with better outcome. The negative lymph-node group had a 5-year survival rate of 50.2%. Conclusion: Transhiatal esophagectomy can be safely used in patients with malnutrition degree that allows the procedure, in those with associated respiratory disorders and in the elderly. It provides considerable long-term survival, especially in the absence of metastases to local lymph nodes. The wider use of neoadjuvant therapy has the potential to further increase long-term survival.

Background: The II Brazilian Consensus on Gastric Cancer by the Brazilian Gastric Cancer Association (ABCG) was recently published. On this occasion, several experts in gastric cancer expressed their opinion before the statements presented. Aim: To present the ABCG Guidelines (part 1) regarding the diagnosis, staging, endoscopic treatment and follow-up of gastric cancer patients. Methods: To forge these Guidelines, the authors carried out an extensive and current review regarding each statement present in the II Consensus, using the Medline/PubMed, Cochrane Library and SciELO databases with the following descriptors: gastric cancer, staging, endoscopic treatment and follow-up. In addition, each statement was classified according to the level of evidence and degree of recommendation. Results: Of the 24 statements, two (8.3%) were classified with level of evidence A, 11 (45.8%) with B and 11 (45.8%) with C. As for the degree of recommendation, six (25%) statements obtained grade of recommendation 1, nine (37.5%) recommendation 2a, six (25%) 2b and three (12.5%) grade 3. Conclusion: The guidelines presented here are intended to assist professionals working in the fight against gastric cancer with relevant and current information, granting them to be applied in the daily medical practice.
