“…presented a case of a patient in whom there was concern for cholangiocarcinoma and who underwent a left trisectionectomy but was ultimately diagnosed with IgG4-related disease 20 . IgG4-related disease can also be initially mistaken for lymphoma, as both diseases can present with lymph node swelling as an initial symptom 21 , 22 . Differentiating IgG4-related disease from cancer is of the utmost importance.…”

“…This obligates a thorough study, with diagnosis and treatment of all malignant-suggestive lesions including surgical options due to benign lesions that simulate malignancy, such as inflammatory pseudotumors. In the literature, the findings in IgG4-related inflammatory pseudotumors are often multi-organic or as a single solid mass [ 7 ], as in this case report.…”

“…There are 3 previously reported cases of IgG4-related disease involving the small bowel. Two presented as bowel wall thickening ( 4 10 ), and 1 as multiple pelvic masses obstructing the bowel lumens ( 3 ). Despite the few reports of IgG4-related disease involving the gastrointestinal tract, there appear to be 2 types of IgG4-related gastrointestinal disease.…”

“…Greater fat infiltration than bowel wall thickening implies that the main pathology lies in the mesentery rather than the gastrointestinal tract ( 11 ). Subserosal plasmacyte infiltration also indicates that the disease originates from the mesentery and involves the gastrointestinal tract ( 10 ). In this case, these factors may have collectively caused periappendiceal fatty strands.…”
