Abstract-Conflicting evidence exists on the relationship between blood pressure (BP) and insulin-like growth factor-1 (IGF-1). We reviewed available articles and pooled extrapolated regression coefficients for the association between BP and total IGF-1 as reported in the literature and included additional data from 912 individuals from the general population. We identified 20 studies including 11 704 subjects. We also measured total IGF-1, insulin-like binding protein-3, and BP in 912 black and white men and women from South Africa (aged 20-70 years). When plotting positive and negative weighed regression coefficients (29 data points) against IGF-1, we found a significant positive relationship (r=0.31; P<0.001; n=11 704) intercepting the 0 point at 191 ng/mL IGF-1, suggesting an inverse BP/IGF-1 relationship in low IGF-1 conditions, and a positive relationship in overtly high IGF-1 conditions. In conclusion, our findings suggest that the relationship between BP and IGF-1 is dependent on, or related to, IGF-1 concentrations, as an expression of direct or reverse causality. Low IGF-1 bioavailability (associated with aging and vascular deterioration), resistance to IGF-1, and the complex interplay between IGF-1 and other vasoactive hormones could mask the vasoprotective functions of IGF-1 in cross-sectional studies or could modify their functions in prospective studies.

Exercise Training in Patients Receiving Maintenance Hemodialysis: A Systematic Review of Clinical Trials

Background: Exercise is not routinely advocated in patients with end-stage renal disease (ESRD) receiving maintenance hemodialysis (HD), compared to best practice in other chronically diseased cohorts. Lack of widespread awareness of the exercise in HD literature may be contributing to these shortcomings of clinical practice. Therefore, our objectives are: (1) to systematically review trials of exercise training involving adult HD patients; (2) to provide empirical evidence that exercise can elicit health-related adaptations in this cohort, and (3) to provide recommendations for future investigations. Method: A systematic review of the literature using computerized databases was performed. Results: According to the 29 trials reviewed, HD patients can safely derive a myriad of health-related adaptations from engaging in appropriately structured exercise regimens involving aerobic and/or resistance training. However, methodological limitations within this body of literature may be partially responsible for minimal advocacy for exercise in this cohort. Conclusions: Robustly designed RCTs with thorough, standardized reporting are required if clinical practice and quality of life of this cohort is to be enhanced through the integration of exercise training and mainstream medical practice. Future trials should demonstrate the clinical importance, and long-term feasibility and applicability of exercise training for this vulnerable patient population.

Exercise training increases exercise capacity of maintenance hemodialysis patients, but the cellular mechanisms responsible for this effect are unclear. We studied the effects of different forms of exercise training (endurance, strength, or a combination where patients underwent about one-half each of the endurance and strength training of the first two groups) on mRNA levels of genes in muscle that may contribute to increased exercise capacity. Before exercise training, muscle mRNA of insulin-like growth factor (IGF)-IEa, IGF-I receptor, IGF-II, IGF-binding protein (IGFBP)-2, and IGFBP-3 were lower in hemodialysis patients than normal controls. After approximately 21 weeks of exercise training, muscle mRNA increased significantly for all of these genes, as well as for IGF-IEc. Muscle mRNA for myostatin, a protein that inhibits skeletal muscle protein synthesis and muscle hypertrophy, decreased. The responses of mRNA to the different types of exercise training generally showed similar significant changes or trends. Muscle IGF-I protein also increased with exercise. Anthropometry, but not dual energy x-ray absorptiometry or bioelectrical impedance, showed a decrease in body fat and an increase in fat-free mass. In conclusion, exercise training in hemodialysis patients induces changes in skeletal muscle mRNA and increases muscle IGF-I protein, which may promote protein anabolism.
