Objectives: This study is part of the development of an International Classification of Functioning, Disability and Health (ICF) Core Set for spinal cord injury (SCI). Its specific objectives were to identify outcome parameters reported in published studies on individuals with SCI in the early post-acute and chronic situation, and to identify and quantify the concepts of the reported parameters using the ICF as a reference. Methods: Electronic searches of Medline, EMBASE, PsycINFO and CINAHL from 2001 to 2005 were carried out. All outcome parameters and their underlying concepts were retrieved from the included studies. These concepts were linked to categories of the ICF using standardized rules. Results: From the 6681 abstracts retrieved, 2205 were randomly selected (33.0%) and 281 studies met the inclusion criteria (12.7%). A total number of 5217 concepts were retrieved from standardized and non-standardized measures, of which 4049 (77.6%) could be linked to 175 different ICF categories: 56 out of 114 Body Functions, 19 out of 56 Body Structures, 62 out of 118 Activities and Participation and 38 out of 74 Environmental Factors categories. Second-level categories reported in 420% of all studies were pain, remunerative employment, health services, systems and policies, school education and higher education. Conclusion:The ICF provides a valuable reference to identify and quantify the concepts of measures focusing on SCI in the early post-acute and chronic situation. The findings show a great diversity in the consequences of SCI and underscore the importance of social participation and environment for people with SCI.

Canadian Occupational Performance Measure performance scale: Validity and responsiveness in chronic pain

Abstract-The construct validity and construct responsiveness of the performance scale of the Canadian Occupational Performance Measure (COPM) was measured in 87 newly admitted patients with chronic pain attending an outpatient rehabilitation clinic. At admission and after 12 wk, patients completed a COPM interview, the Pain Disability Index (PDI), and the RAND 36-Item Health Survey (RAND-36). We determined the construct validity of the COPM by correlations between the COPM performance scale (COPM-P), the PDI, and the RAND-36 at admission. Construct responsiveness was assessed by calculating the correlations between the change scores (n = 57). The COPM-P did not significantly correlate with the PDI (r = 0.260) or with any subscale of the RAND-36 (r = 0.007 to 0.248). Only a moderate correlation was found between change scores of the COPM-P and PDI (r = 0.380) and weak to moderate correlations were found between change scores of the COPM-P and the RAND-36 (r = 0.031 to 0.388), with the higher correlations for the physical functioning, social functioning, and role limitations (physical) subscales. In patients with chronic pain attending our rehabilitation program, the COPM-P measures something different than the RAND-36 or PDI. Therefore, construct validity of the COPM-P was not confirmed by our data. We were not able to find support for the COPM-P to detect changes in occupational performance.

RESUMOObjetivo: Validar empiricamente o core set da CIF para lombalgia e descrever a funcionalidade de uma amostra de pacientes com lombalgia mecânica crônica inespecífica. Métodos: Vinte e nove pacientes de um centro de reabilitação foram avaliados por meio do core set da CIF para lombalgia e pelo questionário RESUMENObjetivo: Validar empíricamente el core set de la CIF para dorsalgia y describir la funcionalidad de una muestra de pacientes con dorsalgia mecánica crónica inespecífica. Métodos: Veintinueve pacientes de un centro de rehabilitación fueron evaluados por medio del core set de la CIF para dorsalgia, y mediante el cuestionario Roland Moris (QRM) y SF-36. Resultados: Todas las categorías de Estructuras del cuerpo del core set se mostraron comprometidas en por lo menos 80% de los pacientes, siendo consideradas validadas. Entre las 19 categorías de Funciones del cuerpo, sólo cuatro estaban comprometidas en menos de 80% de los pacientes, siendo consideradas como no validadas; lo mismo fue observado para 5 de las 29 categorías de Actividades y participaciones, y 5 de las 25 categorías de Factores ambientales. Conclusiones: Las categorías seleccionadas para el core set de la CIF cuanto a dorsalgia fueron consideradas como empíricamente validadas y, en conjunto, permitieron describir la multiplicidad de repercusiones de esa condición de salud sobre la funcionalidad de las personas. Este core set de la CIF sirve para guiar la intervención terapéutica interdisciplinaria.Descriptores: Clasificación internacional de funcionalidad, incapacidad y salud; Calidad de Vida; Dorsalgia; Cuestionarios.
