Identifying and Managing Malnutrition, Frailty and Sarcopenia in the Community: A Narrative Review

Abstract: Malnutrition, frailty and sarcopenia are becoming increasingly prevalent among community-dwelling older adults; yet are often unidentified and untreated in community settings. There is an urgent need for community-based healthcare professionals (HCPs) from all disciplines, including medicine, nursing and allied health, to be aware of, and to be able to recognise and appropriately manage these conditions. “…To date, several barriers have been identified in routine clinical screening of sarcopenia, including the lack of confidence in health care workers in the screening tools, the underestimation of the disease, the lack of specific services to manage sarcopenic patients, or limitations to access these specific services [ 169 , 170 , 171 ]. As a result, sarcopenic dysphagia screening has not been fully introduced in routine clinical practice, albeit recent studies [ 172 , 173 , 174 , 175 ] have emphasized the need for a specific therapeutic intervention including rehabilitation in this condition.…”

“…Both ESPEN and ASPEN identified similar criteria for the diagnosis of malnutrition but with different clinical indicators [16,17]. Following the ESPEN recommendations, subjects at risk of malnutrition should be recognized by validated screening tool criteria, further assessed, and treated accordingly (Table 1).…”

“…Sarcopenia, defined as an age-related generalized loss of skeletal muscle mass, has been related to frailty, overall functional impairment, and poor survival in geriatric population [ 8 10 ]. Although the universal measurements for sarcopenia remain to be established [ 11 ], sarcopenia has been proposed as a predictor of poor prognosis in patients of various chronic diseases, such as cancer [ 12 ], coronary artery disease [ 13 ], and chronic kidney disease [ 14 ].…”
