Identifying and Managing Local Anesthetic Allergy in Dermatologic Surgery

Abstract: True IgE-mediated anaphylaxis to local anesthesia is very rare. Dermatologic surgeons should be aware of the symptoms of anesthetic allergy and its mimickers, as well as how to manage allergic reactions in their clinical practice.

“…Apart from an overdose of the respective substance (maximum doses must be strictly observed -see above), or a reaction to the frequently used additive epinephrine, pseudoallergic (intolerance) reactions can also occur; IgE-mediated anaphylactic reactions are very rare [ 46,47 ] . Local anesthetics of the ester type include procaine, benzocaine, and tetracaine.…”

“…In addition to local reactions at the application site, systemic reactions may also occur. Apart from an overdose of the respective substance (maximum doses must be strictly observed -see above), or a reaction to the frequently used additive epinephrine, pseudoallergic (intolerance) reactions can also occur; IgE-mediated anaphylactic reactions are very rare [ 46,47 ] . Both in case of systemic reactions to LAs as well as all other anesthesiarelated systemic reactions, basal serum tryptase levels should be measured once the reaction has fully subsided in order to rule out occult mastocytosis.…”
