- Mục tiêu: Giảm chức năng hô hấp (CNHH) là một trong những yếu tố tiên lượng xấu trên bệnh phổi tắc nghẽn mạn tính - Chronic obstructive pulmonary disease (COPD). Nghiên cứu nhằm xác định các yếu tố có liên quan đến suy giảm CNHH ở bệnh nhân COPD.
- Phương pháp: Nghiên cứu mô tả các trường hợp bệnh, trên bệnh nhân được chẩn đoán COPD, được theo dõi ngoại trú tại khoa Hô hấp Bệnh viện Nguyễn Tri Phương TpHCM, có hồ sơ bệnh án và phiếu thu thập số liệu đầy đủ thông tin, trong thời gian từ tháng 11 năm 2020 đến tháng 5 năm 2021.
- Kết quả: Trong 131 bệnh nhân 95,4% là nam giới; tuổi trung bình (95% CI) là 64 tuổi (62,6 - 65,1); nghề nghiệp phơi nhiễm được ghi nhận trong 88,5%; 97,7% bệnh nhân hút thuốc lá, trong đó 84,4% hút thuốc ≥ 20 gói - năm; 86,2% phơi nhiễm với hợp chất sinh khói. BMI trung bình là 21,4 (20,8 - 22,0) kg/m2, BMI < 18,5 được tìm thấy trong 21,4% các trường hợp. Sự xuất hiện của đợt cấp trong 12 tháng qua được ghi nhận ở 28,2% bệnh nhân. Phân độ GOLD 2 và nhóm B chiếm ưu thế. Xquang phổi bất thường gặp ở hầu hết các trường hợp (77,9%). Nồng độ FeNO ≥ 25 ppb được ghi nhận ở 37,4% bệnh nhân. CNHH thấp có ý nghĩa thống kê ở nhóm bệnh nhân nam, hút thuốc ≥ 20 gói - năm, phơi nhiễm nghề nghiệp nguy cơ và các hợp chất sinh khói, BMI thấp < 18,5 kg/m2, có đợt cấp trong 12 tháng qua và có bất thường X-quang phổi.
- Kết luận: Cai thuốc lá, phòng tránh ô nhiễm trong nhà, bảo vệ người lao động có nghề nghiệp nguy cơ, nâng cao dinh dưỡng và dự phòng đợt cấp là những can thiệp có thể giúp hạn chế mức độ giảm CNHH trong COPD.
Abstract
- Objective: Lung function decline is one of the poor prognostic factors in chronic obstructive pulmonary disease (COPD). The study aimed to determine the factors related to lung function decline in patients with COPD.
- Methods: This is a case series study, on patients diagnosed with COPD, monitored as outpatient at the Respiratory Department of Nguyen Tri Phuong Hospital, Ho Chi Minh City, with complete medical records and data collection, from November 2020 to May 2021.
- Results: Among 131 patients enrolled in the study, 95.4% were male; mean age (95% CI) was 64 years (62.6 - 65.1); occupational exposure was observed in 88.5% of the patients; there were 97.7% smokers, of which 84.4% smoke ≥ 20 pack - years; 86.2% were exposed to biomass. Mean BMI was 21.4 (20.8 - 22.0) kg/m2 , BMI < 18.5 was found in 21.4%. The occurrence of exacerbations in the past 12 months was observed in 28.2% of the cases. Patients with the GOLD grade 2 and group B were predominant. Abnormal chest x-ray was found in most cases (77.9%). Fraction of exhaled nitric oxide (FeNO) ≥ 25 ppb was observed in 37.4% of the patients. Worsening lung function was statistically significant in male patients; smoking ≥ 20 pack - years; with occupational and biomass exposures, BMI < 18.5 kg/m2 , exacerbations in 12 months passed and abnormal chest X-ray.
- Conclusion: Health education on smoking cessation, prevention of indoor pollution, protection of workers with occupational exposure, improvement of nutrition and prevention of exacerbations are needed for the prevention of respiratory failure in patients with COPD.
