Die Bedeutung dieses Parameters für die Voraussage der lokalen Aktivität von Glutkokortikoiden und die Relevanz für die Gesamtpharmakologische Wirksamkeit dieser Präparate im Zusammenhang mit biopharmazeutischen und pharmakokinetischen Eigenschaften wird kritisch diskutiert. Glucocorticoids are used as effective antiarthritics. They are often administered as intraarticular suspension, in order to prove high local concentrations without systemic side effects. Der Grund für die lokale, intraartikulärer Applikation von schwerlöslichen Glukokortikoid-Kristallsuspensionen liegt dabei in der Möglichkeit, hohe lokale Konzentrationen über einen langen Zeitraum zu gewährleisten und dabei systemische Nebenwirkungen weitgehend zu umgehen. Neben pharmakokinetischen Aspekten müssen die pharmakodynamischen Eigenschaften am Wirkort berücksichtigt werden, wenn vergleichende Aussagen über die Wirkung im Handel befindlicher Kristallsuspensionen angestrebt werden.Die molekular-pharmakologischen Vorgän-ge, die zur antiinflammatorischen Wirkung von Glukokortikoiden führen, sind heute weitgehend bekannt. Es werden heute neben unspezifischen Glukokortikoideffekten, für die sehr hohe Konzentrationen an Wirkstoff notwendig sind, vornehmlich Interaktionen des Glukokortikoids mit spezifischen im Zytosol vorliegenden Rezeptoren verantwortlich gemacht. Dabei wird das Glukokortikoidmolekül nach Aufnahme in die Zelle an einen zytosolischen Rezeptor gebunden. Durch diese Interaktion wird der Glukokortikoid-Rezeptorkomplex aktiviert, in den Kern transportiert und moduliert dort die Aktivität spezifischer Gene. Dies resultiert entweder in der Neubildung oder der Einstellung der Synthese bestimmter Proteine. Ein durch Glukokortikoide induziertes Protein, Lipocortin, hemmt beispielsweise die Synthese von Prostaglandinen und Leukotrienen und ist somit mit für die antiinflammatorische Wirkung der Glukokortikoide verantwortlich (7). The clinical effects in asthma of corticosteroids can be ascribed to their strong anti-inflammatory action. The pathophysiology of asthma is complex. A diversity of mediators and interactions between mediators and between mediators and various cell types enhances and maintains the inflammatory process in the airways resulting in severe asthma. This is a vicious circle. Penetration through the airway epithelium of allergens and other insulting particles trigger release of inflammatory mediators and neurotransmitters, which, by chemotaxis and activation of inflammatory cells, results in enhanced and prolonged mediator release. Several of these mediators possess tissue-toxic properties enhancing the airway inflammation. The glucocorticoids have several points of action affecting both mediator release and accumulation and activation of inflammatory cells, which explains their favourable effect in asthma.

We measured glucocorticoid receptor concentrations in human lung at different stages of alveolar growth. Lung tissue was obtained from 9 surgically aborted or stillborn fetuses of 15 to 28 weeks gestational age, and from 6 infants and children, aged 2 months to 9 years, after lobar resection or at autopsy. Samples were taken from macroscopically healthy areas. Lung histology was performed in all cases. The receptor assay was done by establishing saturation curves for labeled dexamethasone in the absence or presence of a hundredfold excess of unlabeled dexamethasone. Total binding capacity and the dissociation constant were calculated from the saturation curves by the method of Scatchard. The receptor concentration in the fetuses was high (182 +/- 88 fmol/mg prot.), irrespective of gestational age. The lowest concentration (35 fmol/mg prot.) was found in a fetus with pulmonary hypoplasia. In the infants and children the mean receptor concentration was significantly lower (14.6 +/- 9.9 fmol/mg prot.); these included a case of sudden death in which the parenchymal structure was normal.
