Identification of asthma clusters in two independent Korean adult asthma cohorts

Abstract: Asthma is a heterogeneous airway disease with various clinical phenotypes. It is crucial to clearly identify clinical phenotypes to achieve better asthma management. We used cluster analysis to classify the clinical groups of 724 asthmatic patients from the Cohort for Reality and Evolution of Adult Asthma in Korea (COREA), and in 1843 subjects from another independent Korean asthma cohort of Soonchunhyang University Asthma Genome Research Centre (SCH) (Bucheon, Republic of Korea). Hierarchical cluster analysis … Show more

“…Proinflammatory cytokines and matrix metalloproteinases are increased in the lungs of these APN-deficient animals, and APN leads to TNF-a and matrix metalloproteinase-12 suppression (5). Consistent with the hypothesis of low or absent APN in the development of lung emphysema is the observation of endothelial dysfunction in the lungs of adiponectin-knockout mice and the amelioration of the emphysematous phenotype by APN supplementation in these animals, suggesting that APN may play a role in lung maintenance through sustaining endothelial survival (6). These animal data suggest that hypoadiponectinemia plays a critical role in the progression of these structural lung parenchymal changes and the important role of APN in lung maintenance.…”

“…In general, it is difficult to have both of these ideals. Thus, current asthma cluster analyses fall into two groups, large population studies with fewer variables or smaller cohorts with extensive variables (1)(2)(3)(4)(5)(6). The two approaches are complementary and each has merits as we explore asthma heterogeneity.…”

“…In addition to the two phenotypes with severe adult-onset asthma, a recent cluster analysis in 724 Korean patients with asthma also identified a milder adult-onset asthma phenotype [102]. This cluster covered one-third of the total asthma population and had the best lung function.…”

“…Frequent exacerbators are also a well-defined phenotype because of its major source of disease. Such clusters have been replicated in adult and paediatric cohorts in the Western world [24][25][26][27] as well as in Korean and Japanese cohorts 28,29 , with greater concordance in clusters related to severe asthma. Patients on high-dose inhaled corticosteroid therapy made up of clusters 4 and 5 of the adult SARP cohort, often taken together with oral corticosteroid treatment, are usually associated with severe airflow obstruction (Fig.…”
