Iatrogenic Artifacts in Cytology

Abstract: Cel lular morphology may be altered by a variety of iatrogenic factors. These changes may render cytologic diagnosis more difficult and may increase the chance of both false-positive and false-negative interpretations. The authors review a range of physician-related changes involving specimen acquisition, instrumentation, radiation and chemotherapy, ablative procedures, and surgical revisions of anatomic structures in gynecologic and nongynecologic cytology.

“…The cytological changes associated with chemotherapy are generally similar to those seen in response to irradiation; however, the effects are present in fewer cells 81. Chemotherapeutic agents administered intravesically include triethylenethiophosphoramide (thiotepa) and mitomycin C. The cytological changes induced by these two agents are identical and include cellular enlargement with vacuolated cytoplasm and normal nuclear:cytoplasmic ratios.…”

“…Multinucleation and prominent nucleoli may also be noted 80. Bizarre cell shapes and amphophilic cytoplasmic staining are also features of the atypical repair type reaction that is consequent to irradiation 81. It should be noted that the cytological alterations may be present for years after the administration of radiotherapy,81 and a relevant clinical history may not always be provided.…”

“…Other studies also obtained similar results, where the number of artifacts in cytology was higher than cell block (10-13). This was due to Analysis of Cell Block and Cytology Specimen ...... inadequate processing techniques, including the process of specimen collection, preparation of smears, delayed fixation, and staining (14).…”

“…Some of these changes are also evident in urine cytology. Regarding cytology, iatrogenic artifacts are most often related to the use of intravesical immunotherapy, chemotherapy, or radiation 2‐4 . They mostly consist of nonspecific reactive urothelial changes that should not be misinterpreted as malignant.…”
