During recent years, the animal-associated methicillin-resistant Staphylococcus aureus clone ST398 has extensively been studied. The DNA of these isolates turned out to be refractory to SmaI restriction, and consequently, SmaI is unsuitable for subtyping this clone by standard pulsed-field gel electrophoresis (PFGE). Very recently, ST398 DNA was shown to be digested by Cfr9I, a neoschizomer of SmaI. In the present study, we employed Cfr9I PFGE on 100 German and 5 Dutch ST398 isolates and compared their PFGE profiles, protein A gene variable repeat regions (spa types), and types of the staphylococcal cassette chromosome mec (SCCmec). The isolates (from healthy carrier pigs, clinical samples from pigs, dust from farms, milk, and meat) were assigned to 35 profiles, which were correlated to the SCCmec type. A dendrogram with the Cfr9I patterns assigned all profiles to two clusters. Cluster A grouped nearly all isolates with SCCmec type V, and cluster B comprised all SCCmec type IVa and V* (a type V variant first identified as III) carriers plus one isolate with SCCmec type V. Both clusters also grouped methicillin-susceptible S. aureus isolates. The association of the majority of isolates with SCCmec type V in one large cluster indicated the presence of a successful subclone within the clonal complex CC398 from pigs, which has diversified. In general, the combination of Cfr9I PFGE with spa and SCCmec typing demonstrated the heterogeneity of the series analyzed and can be further used for outbreak investigations and traceability studies of the MRSA ST398 emerging clone.

Livestock-associated methicillin-resistant Staphylococcus aureus (LA-MRSA) was detected in 2003 and rapidly became the predominant MRSA clade in the Netherlands. Studies have shown that transmissions are difficult to identify, since this MRSA variant represents a genetically homogenous clade when current typing techniques are used. Here, next-generation sequencing was performed on 206 LA-MRSA isolates to assess the capability of LA-MRSA to be transmitted between humans. The usefulness of single nucleotide variants (SNVs), the composition of the SCCmec region, and the presence of plasmids to identify transmission of LA-MRSA were assessed. In total, 30 presumed putative nosocomial transmission events and 2 LA-MRSA outbreaks were studied; in most cases, SNV analysis revealed that the isolates of the index patient and the contact(s) clustered closely together. In three presumed events, the isolates did not cluster together, indicating that transmission was unlikely. The composition of the SCCmec region corroborated these findings. However, plasmid identification did not support our SNV analysis, since different plasmids were present in several cases where SNV and SCCmec analysis suggested that transmission was likely. Next-generation sequencing shows that transmission of LA-MRSA does occur in Dutch health care settings. Transmission was identified based on SNV analysis combined with epidemiological data and in the context of epidemiologically related and unrelated isolates. Analysis of the SCCmec region provided limited, albeit useful, information to corroborate conclusions on transmissions, but plasmid identification did not. IMPORTANCE In 2003, a variant of methicillin-resistant Staphylococcus aureus (MRSA) isolated from pigs was also found in pig farmers inFrance and the Netherlands. Soon thereafter, this livestock-associated MRSA (LA-MRSA) was identified in many other countries. Transmission of LA-MRSA between humans, particularly in the health care setting, is regarded to occur sporadically. Moreover, studies that describe LA-MRSA transmission used molecular characterization of isolates with limited discriminatory power, making the validity of the conclusion that transmission occurred questionable. In our study, we sequenced the complete genomes of 206 LA-MRSA isolates, obtained from more than 30 presumed LA-MRSA transmission events. Analysis of the data showed that transmission of LA-MRSA between humans had indeed occurred in more than 90% of these events. We conclude that transmission of LA-MRSA between humans does occur in Dutch health care settings; therefore, a decision to discontinue the search and destroy policy for LA-MRSA should be taken with caution. Sequence type 398 (ST398) Staphylococcus aureus, frequently carried by livestock, has caused severe human infections and often carries transmissible antibiotic resistance genes. Among methicillin-susceptible S. aureus isolates colonizing Dallas County Jail detainees, 13.2% were ST398, spa type t571, and were genetically similar to human colonization isolates from New York, Chicago, and the Dominican Republic. Sequence type 398 (ST398) methicillin-resistant Staphylococcus aureus (MRSA) is often transmitted among livestock and animal handlers but is uncommonly associated with human disease in the United States (1). We unexpectedly identified ST398/t571 methicillin-sensitive S. aureus (MSSA) as the most common genetic background among MSSA isolates colonizing detainees in the Dallas County Jail. In this study, we compared the pulsed-field gel electrophoresis (PFGE) pulsotypes of isolates from the Dallas jail and from a single patient in Chicago with those of strains isolated in previous studies.Recognized first in Europe (2-9), ST398 MRSA has been isolated in the United States (10), Canada (11), and Latin America (12). ST398 methicillin-susceptible S. aureus (MSSA) isolates have been recovered from pigs (13), humans (14-17), and retail meat (18). Asymptomatic human colonization and infection with ST398 MSSA are rare in the United In January 2009, we screened the anterior nares and the hands of 928 detainees, housed in 68 divisions, called tanks, in the Dallas County Jail, for S. aureus carriage. Tanks had a capacity of 24 to 36 detainees. Detainees can interact with others within a single tank but have little interaction with those in other tanks. The study was approved by the Institutional Review Boards at the University of Chicago Medical Center and the University of Texas-Southwestern Medical Center. Recovered MSSA isolates were genotyped from subjects housed in a stratified random sample of 26 of the 68 tanks; all MRSA isolates were genotyped.In a separate study of S. aureus colonization among patients and household contacts (24), we also identified one patient in Chicago with ST398/t571 MSSA colonization.Multilocus sequence typing (MLST) (25), spa typing (26), PCR for the Panton-Valentine leukocidin (PVL) genes (27), and PFGE using Cfr9I digestion (22, 28) were performed. Antimicrobial susceptibilities were determined by automated testing (29).Among the 345 subjects in the 26 selected tanks, 110 (31.9%) carried MSSA. Of 158 MSSA isolates identified, 152 were available, and 34 MLST types were identified. Twenty isolates (13%) were ST398, the most common ST; all lacked PVL genes.
