Abstract. The present study describes circular RNA (circRNA) profiles in three pairs of hepatocellular carcinoma (HCC) tissues and the corresponding adjacent non-tumorous tissues (NTs) by microarray. circRNA is a type of endogenous RNA that serve a crucial role in disease development and aberrantly express in a number of types of cancer. In the present study, 3 paired HCC tissues and paired adjacent NTs were collected from HCC surgical specimens from 3 hepatitis B virus-infected patients with HCC. With abundant and varied probes accounting for 5,396 circRNAs, a large number of circRNAs are able to be quantitatively determined. Based on the microarray data, 222,567,556 upregulated circRNAs and 125,439,219 downregulated circRNAs were identified respectively. Further analysis revealed 24 upregulated and 23 downregulated significantly circRNAs (fold-change ≥2; P≤0.05) in HCC tissues compared with NTs. By means of computer analysis and database inquiring, the microRNA (miRNA) response elements associated with the abnormally expressed circRNAs were annotated. The present study showed novel evidence determining genome-wide circRNA expression patterns in HCC using microarray analysis. The results demonstrated that clusters of circRNAs were aberrantly expressed in HCC compared with NTs. These circRNAs may be involved in the occurrence and development of HCC. Therefore, the results of the present study may provide a novel approach for improving the understanding of the molecular basis of HCC. Furthermore, the identified circRNAs may be potential biomarkers for the diagnosis of HCC.

Abstract. Circular RNAs (circRNAs) may serve as biomarkers for a potentially non-invasive diagnosis of cancer. To understand their diagnostic performance, a systematic meta-analysis of the published literature was conducted to review the diagnostic efficiency of circRNAs in patients with cancer. Eligible studies published up to November 30, 2017, on PubMed and EMBASE, were selected for the meta-analysis. All studies were carefully and independently reviewed by two researchers based on their titles and abstracts, following which full texts were perused for potential eligibility. All statistical analyses were performed by STATA 13.0 statistical software and Meta-DiSc 1.4. A total of 10 eligible studies were included. The pooled diagnostic odds ratio was 7.265. The pooled sensitivity was 0.708 and the pooled specificity was 0.722. The positive likelihood and negative likelihood ratios were 2.483 and 0.372, respectively. The area under the curve was 0.793. circRNA was determined to be a notably effective assistant diagnostic biomarker for cancer. IntroductionCancer is a major public health problem globally. Cancer is a class of diseases that undergoes uncontrollable cell proliferation and differentiation. Based on the 2015 cancer statistics, it is currently the second leading cause of mortality in numerous countries (including China, Europe and the USA), and is expected to surpass heart diseases as the leading cause of mortality in the near future (1). Although the risk of succumbing to cancer has decreased by ~20% from its maximum in 1991-2011 (1), it must be diagnosed with high sensitivity and specificity in order to determine the appropriate therapy and prognosis. Recently, a number of biomarkers with diagnostic and prognostic potential value have been demonstrated in numerous cancer types, including the tumor markers human epididymis secretory protein 4 and cancer antigen 125 in endometrial (2) and ovarian cancer types (3). Additionally, mutant genes have been used in the selection of an appropriate therapy, including epidermal growth factor receptor mutation in non-small cell lung cancer (4), Kirsten rat sarcoma viral oncogene homolog in colorectal cancer (5) and v-raf murine sarcoma viral oncogene homolog B1 mutation in melanoma (6); however, reliable and convenient biomarkers are required to evaluate the diagnostic and prognostic significance of different cancer types.Classic biomarkers present with potentially limiting factors, including cost, availability and reproducibility (7). Background/Aims: Circular RNAs (circRNAs), a novel class of noncoding RNAs, have been found to be dysregulated in various cancers. However, the clinical application value of these circRNAs in digestive system cancers remains to be clarified. We aimed to comprehensively explore the potential role of circRNAs as diagnostic indicators in digestive system malignancies. Methods: Relevant studies were systematically retrieved from PubMed, Web of Science and the Cochrane Library. The data that were required to complete 2 × 2 contingency tables were obtained from the included studies. Stratified analyses by cancer type, sample size and publication year were performed. Results: Thirteen studies with 2,276 individuals were included in the meta-analysis. The pooled sensitivity and specificity of circRNAs in the diagnosis of digestive system malignancy were 0.72 [95% confidence interval (CI): 0.65-0.77] and 0.77 (95% CI: 0.72-0.81), respectively. The overall positive likelihood ratio was 3.09 (95% CI: 2.64-3.62), and the overall negative likelihood ratio was 0.37 (95% CI: 0.31-0.44). The pooled diagnostic odds ratio was 8.38 (95% CI: 6.86-10.25), and the overall area under the curve was 0.81 (95% CI: 0.77-0.84), indicating good discriminative ability of circRNAs as biomarkers for digestive system malignancy. Conclusion: circRNAs distinguish patients with digestive system cancer from controls with relatively high diagnostic accuracy. circRNAs may be used as potential biomarkers for the diagnosis of digestive system malignancy.
