Mycobacterium fortuitum: an iatrogenic cause of soft tissue infection in surgery

Abstract: A high index of suspicion based on clinical presentation is essential to diagnose M. fortuitum as a cause of soft tissue infection. Treatment involves aggressive surgical debridement and administration of combination antibiotics based on sensitivity, which should be continued for a period that will ensure complete healing and prevent recurrence.

“…M.fortuitum responds to antibiotics like amikacin, quinolones, doxycycline and sulphamethaxole. As per latest studies clarithromycin, cefoxitin and imipenem were useful for treatment against RGM [17][18][19]. Almost all the patients in our study were cured with a combined approach of drainage and clarithromycin based combination therapy.…”

“…Infections with atypical mycobacteria have been primarily reported after laparoscopic procedures [9,10]. This is because, unlike open surgery, the instruments used here have a layer of insulation that restricts the use of the autoclave in the sterilization process.…”

“…18 There has also been a report from our institution on the isolation of M. fortuitum from surgical specimens of patients with post operative wound infections, injection abscesses, skin and soft tissue infections. 15 Although there are many reports from India, the exact disease burden of NTM infections still remains unclear in India. These infections are under diagnosed in many laboratories due to lack of facilities and expertise.…”
