Mycobacterium abscessus Infection in Cystic Fibrosis

Abstract: We present a case of a patient with cystic fibrosis who was thought to be colonized with Mycobacterium abscessus for 13 yr prior to developing clinically apparent mycobacterial infection. However, histologic evidence indicated that invasive mycobacterial disease was present from the onset. While accepting that chronic endobronchial colonization with atypical mycobacteria may occur in patients with cystic fibrosis, the repeated isolation of mycobacteria from the sputum of these patients should alert the clinici… Show more

“…The potential for patients with CF to have "indolent infection" by NTM, including M. abscessus, has previously been described in case series but is not well defined or universally accepted (8,12,14,31,32). Over the period of analysis, spanning ORIGINAL RESEARCH 1 year before and 3 years after the initial positive culture, we were unable to demonstrate a difference in FEV 1 decline between subjects with transient or persistent infection.…”

“…Finally, another disturbing finding is the extremely heterogeneous clinical presentation of MABSC disease, extending from totally asymptomatic infection to fulminant forms in some patients, whether or not they had undergone transplantation [7][8][9][10]. This observation suggests a particular susceptibility to MABSC infection in some patients.…”

“…Chronic MABSC infection is associated with a greater decline of lung function [6]. MABSC can cause severe pseudotuberculous lung disease [8], in some cases after following an indolent course for a number of years [9]. It may cause disseminated, sometimes fatal infections in lung transplant recipients [10,11].…”

“…It is also a pulmonary pathogen in patients with cystic fibrosis (Cullen et al, 2000;Fauroux et al, 1997;Olivier et al, 2003;Sermet-Gaudelus et al, 2003). In some instances, isolation of M. abscessus from the sputum is not associated with progressive lung infection and may be the result of transient airway colonization (Griffith et al, 2007;Jönsson et al, 2007).…”
