In vivoprobe-based confocal laser endomicroscopy in amiodarone-related pneumonia

Abstract: Probe-based confocal laser endomicroscopy (pCLE) allows microscopic imaging of the alveoli during bronchoscopy. The objective of the study was to assess the diagnostic accuracy of pCLE for amiodarone-related pneumonia (AMR-IP).Alveolar pCLE was performed in 36 nonsmoking patients, including 33 consecutive patients with acute or subacute interstitial lung disease (ILD), of which 17 were undergoing treatment with amiodarone, and three were amiodarone-treated patients without ILD.Nine out of 17 patients were diag… Show more

“…Endogenous fluorescence allows elastin and tobacco-tar laden macrophages (in smokers) to be identified without the use of intravenous contrast (10). FCFM has already been used to identify malignancy, acute rejection in lung transplantation, amiodarone lung and pulmonary alveolar proteinosis in pilot data (11)(12)(13).…”

“…Previous studies used pCLE to investigate interstitial lung disease, alveolar proteinosis, drug-induced (amiodarone) pneumonitis, lung cancer [15,16,17,18] and, very recently, chronic obstructive pulmonary disease [9]. To the best of our knowledge, this is the first study to investigate the potential clinical usefulness of pCLE in the diagnosis of PJP in HIV+ patients.…”

“…Previous studies have reported a minor risk of pneumothorax and chest pain [15] or mild bleeding [19] following pCLE. This was not the case in any of the patients included in our study.…”

“…The detachment of the imaging microprobe from other components of the imaging system allows in situ imaging of organs and tissues within a body, which cannot be performed with conventional microscopy. In the last years, the unique capability of real-time, in situ microscopic imaging has led to development of FCM-based approaches for clinical diagnosis in gastrointestinology (Inoue et al, 2005; Kiesslich and Neurath, 2005; Goetz et al, 2006; Anandasabapathy, 2008), pulmonology (Thiberville et al, 2009; Salaun et al, 2013), and urology (Wu et al, 2011). …”
