In Vivo Evolution of CTX-M-215, a Novel Narrow-Spectrum β-Lactamase in an Escherichia coli Clinical Isolate Conferring Resistance to Mecillinam

Abstract: Here we report a novel narrow-spectrum β-lactamase CTX-M-215 identified in an Escherichia coli clinical isolate in China and conferring high-level resistance to mecillinam but not to cefotaxime. CTX-M-215 differed from CTX-M-125, a CTX-M-ESBL by an N132D substitution, which decreased the hydrolytic activities towards penicillins and cephalosporins except for mecillinam. High similarity was observed between CTX-M-215- and CTX-M-125-bearing plasmids, carried by different isolates in the same patient, indicating … Show more

“…In the present work, 4.33% (25/507) of the strains isolated had more than one gene from the bla CTX–M group, which should be highlighted regardless of the low percentage due to the potential for the development of other enzyme recombinants by these strains in the future ( Yin et al, 2020 ; Leão et al, 2021 ).…”

“…11 Despite its clinical significance, CTX-M is not limited to clinical settings as has been detected in a variety of veterinary, 12 agricultural, 13,14 and environmental settings. 15,16 As new CTX-M alleles, subgroups, and clusters are continuously being identified in bacterial isolates, [17][18][19][20] there is a need to characterize alleles at the population-level. Surveillance of ARGs in populations can be done through wastewater-based epidemiology (WBE).…”

“… 8 In addition, most ESBL-producing Enterobacterales remain susceptible to mecillinam. 9 , 10 Some ESBLs, such as bla CTX-M-215 , have been described to confer a high level of resistance to mecillinam, 11 but their prevalence has remained low. In the literature, there are some data on the susceptibility of CRE to mecillinam, but these studies mainly focused on few bacterial species ( E. coli or Klebsiella pneumoniae ), few carbapenemase types (OXA-48 or NDM) or on a limited number of strains.…”
