In Vitro Evaluation of Allergen Potencies of Commercial House Dust Mite Sublingual Immunotherapy Reagents

Abstract: PurposeThe clinical efficacy of allergen-immunotherapy is known to be dose dependent. However, optimal maintenance dosage has not yet been determined for sublingual immunotherapy (SLIT). Furthermore, since companies adopt their own units for expression of allergenicity, the allergen concentrations of individual reagents cannot be compared easily. We sought to measure and compare the allergenicities of 3 commercially available house dust mite (HDM) SLIT regents and a subcutaneous immunotherapy reagent.MethodsWe… Show more

“…These observed differences, especially between products from different manufacturers, are not limited to minor allergens though. As already described in the literature, BP allergen products also differ greatly with regard to their main quality parameters total allergenic activity, total protein and Bet v 1 content . Regarding SCIT products, these parameters could only be determined in intermediates, prior to chemical modification and/or absorption.…”

“…Our method proved to be compatible with glycerol and phenol, the most common excipients in the formulation of skin prick test and sublingual immunotherapy (SLIT) products . In addition, the sensitivity of our method allows the analysis of highly diluted extracts (0.75 μg Der p 1/ml and 0.63 μg Der p 2/ml), equivalent to the lowest concentrated products cited in the literature . Further investigation is needed to confirm compatibility with other AIT products (e.g.…”

“…If SLIT is unsuccessful, the diagnosis ought to be critically reviewed. If no competing allergies can be detected, a change of the preparation can be considered, since the composition of different allergen extracts varies depending on the manufacturer [ 367 ].…”
