The goal of this study was to determine the effect of assembly load and local assembly environmental conditions on the fretting corrosion of modular femoral stem tapers. Femoral head/ taper assemblies in both similar (CoCrMo/CoCrMo) and mixed (CoCrMo/Ti-6Al-4V) alloy combinations were evaluated using an electrochemical test method. Specimens were assembled under impact loading and by hand, in both wet and dry conditions. Incremental cyclic loads ranging from 89 to 5,340 N were applied at a frequency of 3 Hz in Ringer's solution at ambient temperature. During the test, both the open circuit potential (OCP) and fretting current (i fret ) were measured using a saturated calomel electrode (SCE) and counter electrode, respectively. The results were comparable for both mixed and similar alloy couples. Decreases in OCP and increases in i fret (indicators of oxide film fracture and repassivation) were seen with increasing load magnitude, often occurring at loads well below those expected clinically. OCP at the 5,340 N cyclic load ranged from À30.4 to À103.7 mV versus SCE for similar alloy couples, and À19.1 to À181.4 mV versus SCE for mixed alloy couples. Mean peak fretting currents ranged from 0.84 to 1.42 mA and 1.06 to 3.12 mA for similar and mixed alloy couples, respectively. The larger current magnitudes and more negative shifts in OCP for mixed alloy couples indicate the difference in oxide film fracture behavior between titanium and cobalt alloys. The load at which OCP began to drop (onset of fretting) was dependent upon the assembly conditions for both material couples. Specimens assembled with impact loads in air showed the highest resistance to fretting. The results of this study indicate that the assembly load and the environment both play a role in the initial stability of modular hip taper connections.

Background Modularity of the femoral head-neck junction provides increased intraoperative flexibility to the surgeon. Complications of this modularity include damage to the trunnion, with subsequent bone and/or soft tissue loss from adverse reactions to metal debris. Case Description We describe two cases of severe metalinduced osteolysis and soft tissue damage requiring revision 10 and 13 years following implantation of a unipolar endoprosthesis. Damage to the trunnion resulted in severe acetabular and trochanteric osteolysis and soft tissue loss requiring complex revision surgery. Literature Review Several reports have shown the trunnion, the head-neck interface, and the neck-stem couple as the causes of this early failure secondary to metal ion release from mechanical fretting corrosion or from crevice corrosion at these modular interfaces. These reports have been in association with a total hip prosthesis rather than a unipolar endoprosthesis. Revision of a unipolar endoprosthesis is most commonly attributable to stem loosening or acetabular erosion from the large femoral head articulating on the host acetabular cartilage and not owing to failure of the trunnion. Purposes and Clinical Relevance Trunnion damage resulting in a severe reaction to metal debris with acetabular osteolysis, erosion of the greater trochanter, and loss of the abductor mechanism can occur years after implantation of a cementless unipolar endoprosthesis. This raises questions regarding long-term safety of the modular interface of a contemporary cementless stem and a large-diameter unipolar head. We recommend long-term followup of patients with a unipolar endoprosthesis as early recognition and treatment are required to avoid a potentially complex revision.

Polyethylene glycolated bisphenol A (PEGBPA), a hydrophilic-hydrophobic compound, was synthesized and applied onto polyester fabric using a pad-dry-cure method to impart a durable hydrophilic property. After coating and heat fixation, the treated fabrics were evaluated for wettability by measurement of the net moisture regain and wicking distance, and their surfaces were characterized by SEM and ATR/FTIR spectroscopy. The durability of the treated fabrics were tested by ten standard repeat washings and samples were characterized for their wettability after each wash. The surface properties of the fabric changed from hydrophobic to hydrophilic after heat treatment with the coating agent PEGBPA. Using the degree of increased moisture regain and wicking distance of the treated fabrics as a guide, optimal treatment was attained by coating with 10-20 g/ L PEGBPA followed by 160-180 C heat treatment for 3 min. Wash fastness evaluations, coupled with SEM and ATR/FTIR analyses, showed that PEGBPA exhibited good adhesion onto the PET surface and was capable of withstanding repeated washings. It was concluded that the coating adhesion solely depended on physical aspects, such as hydrophobic-hydrophobic interactions between the bisphenol A segment of PEGBPA and the PET aromatic segment.
