“…In our review of the literature, interposition vein grafts are associated with a 36% failure rate in the treatment of HHS (Table 1). 2,9,[13][14][15][16][17][18][19] It is well known in the cardiac literature that arterial grafts maintain superior patency over the traditional saphenous vein grafts for coronary artery bypass surgery. [5][6][7][8] Although the short-term patency of the radial artery graft and saphenous vein graft are comparable, the radial artery is superior in the medium-and long-term patency rates.…”

“…The clinical presentation of HHS can be seen with intermittent findings and symptoms that appear following trauma. Typically, ischemia of the second, third, fourth or fifth fingers of the dominant hand develops in male patients [5]. The ulnar artery provides blood flow to the majority of the fingers.…”

“…Our initial evaluation of unilateral versus bilateral symptoms is determined on account of the demonstration that most symptomatic HHS is unilateral in the dominant hand (although bilateral pathology has been reported, vasospastic disorders such as Raynaud's and CREST are commonly bilateral. 4 It is not necessary to obtain segmental arm pressures when a clearly visible pulsatile mass indicates aneurysmal HHS to be the likely diagnosis, as seen in the case of Patient 2. When interpreting the PPG waveforms, if the abnormal waveforms are diffuse and across all digits, it is more likely that the cause of HHS is due to the lack of adequate collateralization, than embolic events.…”
