Hypomethylation of NLRP3 gene promoter discriminates glucocorticoid‐resistant from glucocorticoid‐sensitive idiopathic nephrotic syndrome patients

Abstract: To assess whether NLRP3 gene promoter methylation was able to discriminate glucocorticoid (GC)‐resistant from GC‐sensitive idiopathic nephrotic syndrome (INS), patients with minimal change disease (MCD) or focal segmental glomerulosclerosis (FSGS), we measured the methylation level of NLRP3 promoter in DNA from peripheral blood cells of 10 adult patients with GC‐resistant FSGS already in hemodialysis and 18 patients with GC‐sensitive INS (13 MCD/5 FSGS) and in 21 pediatric patients with INS with MCD/FSGS befor… Show more

“…Inflammasome involvement has been suggested. In peripheral blood mononuclear cells (PBMCs) from patients with glucocorticoid-resistant nephrotic syndrome, NLRP3 protein co-localized with ASC protein, which could indicate inflammasome activation [ 285 ]. In this study, methylation of the NLRP3 gene promotor, which should result in decreased NLRP3 gene expression, was significantly higher in PBMCs from patients with glucocorticoid-sensitive nephrotic syndrome.…”

“…Circulating GRβ levels have been found to be associated with GC resistance in rheumatoid arthritis, SLE, and asthma ( 64 – 66 ). Hypomethylation of NLRP3 gene promoter discriminates glucocorticoid-resistant from GC-sensitive idiopathic nephrotic syndrome patients ( 67 ). P53 interacts with GR to promote anti-inflammatory pathways and patients with rheumatoid arthritis who did not respond to GC treatment showed reduced p53 expression levels in blood mononuclear cells ( 68 ).…”

“…However, there is a wide variation as some patients respond to steroids, whereas others remain steroid-resistant. 6 In clinical practice, patients are classified into steroid-sensitive nephrotic syndrome (SSNS), steroid-resistant nephrotic syndrome (SRNS), based on steroid responsiveness. 7 Over 80% of patients accomplish complete remission by prednisolone therapy.…”
