We reviewed 33 original research studies assessing brain perfusion, using consensus guidelines from a “white paper” issued by the International Society for Magnetic Resonance in Medicine Perfusion Study Group and the European Cooperation in Science and Technology Action BM1103 (“Arterial Spin Labelling Initiative in Dementia”; https://www.cost.eu/actions/BM1103/). The studies were published between 2011 and 2023 and included participants with subjective cognitive decline plus; neurocognitive disorders, including mild cognitive impairment (MCI), Alzheimer’s disease (AD), frontotemporal lobar degeneration (FTLD), dementia with Lewy bodies (DLB) and vascular cognitive impairment (VCI); as well as schizophrenia spectrum disorders, bipolar and major depressive disorders, autism spectrum disorder, attention-deficit/hyperactivity disorder, panic disorder and alcohol use disorder. Hypoperfusion associated with cognitive impairment was the major finding across the spectrum of cognitive decline. Regional hyperperfusion also was reported in MCI, AD, frontotemporal dementia phenocopy syndrome and VCI. Hypoperfused structures found to aid in diagnosing AD included the precunei and adjacent posterior cingulate cortices. Hypoperfused structures found to better diagnose patients with FTLD were the anterior cingulate cortices and frontal regions. Hypoperfusion in patients with DLB was found to relatively spare the temporal lobes, even after correction for partial volume effects. Hyperperfusion in the temporal cortices and hypoperfusion in the prefrontal and anterior cingulate cortices were found in patients with schizophrenia, most of whom were on medication and at the chronic stage of illness. Infratentorial structures were found to be abnormally perfused in patients with bipolar or major depressive disorders. Brain perfusion abnormalities were helpful in diagnosing most neurocognitive disorders. Abnormalities reported in VCI and the remaining mental disorders were heterogeneous and not generalisable.

Background: Increasing evidence suggests that both amyloid-β metabolism disorders in the liver and cerebral hypoperfusion play an important role in the pathogenesis of Alzheimer’s disease (AD). However, the relevance of liver function alterations to cerebral blood flow (CBF) of patients with AD remains unclear. Objective: We aimed to investigate the associations between liver function changes and CBF of patients with AD. Methods: We recruited 17 patients with sporadic AD. In addition to physical and neurological examinations, detection of AD biomarkers in cerebrospinal fluid by enzyme-linked immunosorbent assay and CBF assessment by arterial spin labeling sequence of magnetic resonance image scans as well as measure of liver function markers in serum by routine laboratory testing were conducted. Neuropsychological tests were evaluated, including Mini-Mental State Examination and Montreal Cognitive Assessment. Linear and rank correlations were performed to test the associations of liver function alterations with regional CBF of AD. Results: We found that liver function markers, especially total protein, the ratio of albumin to globin, globin, alkaline phosphatase, and aspartate aminotransferase were significantly associated with regional CBF of AD patients. Conclusions: These findings demonstrated significant associations between perfusion in certain brain regions of AD and alterations of liver function markers, particularly proteins and liver enzymes, which might provide implications to the pathogenesis and treatment of AD.

Multi-modal feature selection with self-expression topological manifold for end-stage renal disease associated with mild cognitive impairment

<abstract> <p>Effectively selecting discriminative brain regions in multi-modal neuroimages is one of the effective means to reveal the neuropathological mechanism of end-stage renal disease associated with mild cognitive impairment (ESRDaMCI). Existing multi-modal feature selection methods usually depend on the <italic>Euclidean</italic> distance to measure the similarity between data, which tends to ignore the implied data manifold. A self-expression topological manifold based multi-modal feature selection method (SETMFS) is proposed to address this issue employing self-expression topological manifold. First, a dynamic brain functional network is established using functional magnetic resonance imaging (fMRI), after which the betweenness centrality is extracted. The feature matrix of fMRI is constructed based on this centrality measure. Second, the feature matrix of arterial spin labeling (ASL) is constructed by extracting the cerebral blood flow (CBF). Then, the topological relationship matrices are constructed by calculating the topological relationship between each data point in the two feature matrices to measure the intrinsic similarity between the features, respectively. Subsequently, the graph regularization is utilized to embed the self-expression model into topological manifold learning to identify the linear self-expression of the features. Finally, the selected well-represented feature vectors are fed into a multicore support vector machine (MKSVM) for classification. The experimental results show that the classification performance of SETMFS is significantly superior to several state-of-the-art feature selection methods, especially its classification accuracy reaches 86.10%, which is at least 4.34% higher than other comparable methods. This method fully considers the topological correlation between the multi-modal features and provides a reference for ESRDaMCI auxiliary diagnosis.</p> </abstract>
