Pendahuluan: Impending eclampsia merupakan masalah yang serius dalam kehamilan karena komplikasi-komplikasi yang dapat timbul baik pada ibu maupun pada janin. Salah satu cara terbaik untuk meningkatkan keselamatan ibu dan bayi pada pasien dengan impending eklampsia adalah dengan dilakukan pembedahan caesar. Laporan Kasus: Pada kasus ini, akan dibahas lebih lanjut terkait wanita 39 tahun dengan G4P3A0 hamil 39 minggu dengan impending eclampsia. Pasien ini memiliki status fisik ASA III-E dan dilakukan tindakan sectio caesarea transperitoneal emergency dengan teknik anestesi regional subarachnoid block. Bayi lahir berjenis kelamin perempuan dengan APGAR score 7–8–9. Diskusi: Anestesi regional subarachnoid block dipilih karena mempunyai banyak keuntungan seperti kesederhanaan teknik, onset yang cepat, resiko keracunan sistemik yang kecil, blok anestesi yang baik, pencegahan perubahan fisiologi dan penanggulangannya sudah diketahui dengan baik. Kesimpulan: Diagnosis dan manajemen yang tepat pada ibu hamil dengan impending eclampsia sangat penting untuk kelangsungan hidup pasien. Pemilihan jenis anestesi subarachnoid block dipilih sesuai dengan kondisi klinis pasien dengan memperhitungkan segala aspek keuntungan, kerugian dan aspek medis lainnya. Management Anesthesia Subarachnoid Block for Patient with Impending Eclampsia Abstract Introduction: Impending eclampsia is a serious problem in pregnancy because of complications that can arise both for the mother and the fetus. One way to speed up handling and improve the safety of mother and baby in patients with impending eclampsia is by caesarean section. Case Report: In this case, we will discuss about 39-year-old woman with G4P3A0 39 weeks pregnancy with impending eclampsia. This patient has ASA III-E physical status and performed transperitoneal emergency sectio caesarea under regional subarachnoid block anesthesia. The baby is female, born alive without abnormalities and has 7-8-9 APGAR score. Discussion: Regional subarachnoid block anesthesia was chosen because it has many advantages such as simple technic, rapid onset, a small risk of systemic poisoning, good anesthesia block, prevention of physiological changes, and its handling are well known. Conclusion: Proper diagnosis and management of pregnant women with impending eclampsia is very important for patient survival. The choice of subarachnoid block anesthesia is chosen according to the clinical condition of the patient by considering all aspects of the advantages, disadvantages, and other medical aspects.

In a country like India, where maternal mortality rate is still very high despite progress and development which has consistently been made in the health services, a big proportion is still deprived of it. Hypertension is the most common medical problem encountered during pregnancy. This study was undertaken to compare the efficacy of labetalol versus methyldopa in the treatment of pre-eclampsia and prevention of further complications. The present study had been carried out on antenatal booked 110 cases between 20 – 40 years of age who developed hypertension after 20 weeks period of gestation with or without oedema & proteinuria or both, attending antenatal OPD or Emergency at Govt General Hospital, Nizamabad, from 1st December 2017 to 31may 2018. : Incidence of PIH in the present study is 7.23% in the booked population. Side effects of drugs were reported more in methyldopa group as compared to labetalol. Adverse foetal outcomes, especially IUGR and pre term babies are more in methyldopa group as compared to labetalol group. Emergency lscs cases are 14 out of 55 in labetalol group,22 out of 55 in methyldopa group were noted. Most common indication for emergency LSCS in both the groups was uncontrolled hypertension, but cases are more in methyldopa group. in Labetalol group 7 out of 55 underwent EL LSCS, in methyldopa group 6 out of 55 underwent LSCS. Total no of caesarean sections in labetalol group are 21 out of 55(32.7%),whereas in methyldopa group28 out of 55 (50.9%), so operative interference is more in methyldopa group when compared to labetalol group.: It concludes that labetalol is suitable for use during pregnancy to reduce maternal and fetal side-effects, so it has efficient hypotensive action.

Arteriální hypertenze v těhotenství představuje různé typy onemocnění, mezi které řadíme preexistující hypertenzi, preeklampsii/eklampsii, preeklampsii nasedající na preexistující hypertenzi a gestační hypertenzi. Arteriální hypertenze patří v graviditě mezi hlavní příčiny mateřské i fetální morbidity a mortality. Cílem naší studie bylo určit typ, prevalenci a průběh arteriální hypertenze u souboru gravidních kontrolovaných v poradně pro těhotné s kardiovaskulárními chorobami. Šlo o retrospektivní studii u gravidních žen sledovaných v poradně od ledna 2006 do prosince 2009. Celkem 52 žen (13 %) s arteriální hypertenzí bylo vybráno ze souboru 408 pacientek se 413 graviditami. U 35 těhotných (67 %) šlo o preexistující hypertenzi, u sedmi (13,5 %) o gestační hypertenzi, u dalších sedmi (13,5 %) o hypertenzi při fenoménu bílého pláště. Preeklampsie byla diagnostikována u tří těhotných (6 %). Až na jednu výjimku (císařský řez ve 36. týdnu gravidity) byly všechny gravidity ukončeny v termínu spontánním porodem. Při pravidelných kontrolách a terapii má těhotenství u žen s lehkou až středně významnou arteriální hypertenzí ve většině případů příznivý průběh.
