Elevated blood pressure (BP) is a growing burden worldwide, leading to over 10 million deaths each year. In Cote d’Ivoire, high BP is the first risk factor for stroke and myocardial infarction. May Measurement Month (MMM) is a global initiative of the International Society of Hypertension aimed at raising awareness of high BP and to act as a temporary solution to the lack of screening programmes worldwide. An opportunistic cross-sectional survey of volunteers aged ≥18 was carried out in May 2017. BP measurement, the definition of hypertension and statistical analysis followed the standard MMM17 protocol. A Coordinators Team was created. All doctors in cardiology training of Abidjan Institute of Cardiology ( N = 72) were briefed to carry out the MMM study as investigators, and 55 sites were created. These were major urban transport stations, supermarkets and markets, and churches. BP was calculated from the mean of the 2nd and 3rd readings, and hypertension was defined as a systolic BP of at least 140 mmHg and/or a diastolic BP of at least 90 mmHg or being on BP-lowering treatment. We screened 24 563 individuals during MMM17, with mean age of 37 years. After multiple imputations, 5015 (20.4%) had hypertension. Of individuals not receiving anti-hypertensive medication, 3943 (16.8%) were hypertensive. Of individuals receiving anti-hypertensive medication, 583 (54.4%) had uncontrolled BP. MMM17 was the largest BP screening campaign undertaken in Cote d’Ivoire. MMM allowed us to identify many undiagnosed and inadequately treated hypertension cases and could usefully lower the burden attributed to increased BP. MMM should be continued.

SummaryBackgroundUncontrolled hypertension remains an important issue in daily clinical practice worldwide. Although the majority of patients are treated in primary care, most of the data on blood pressure control originate from populationbased studies or secondary healthcare.ObjectiveThe aim of this study was to evaluate the frequency of uncontrolled hypertension and associated risk factors among hypertensive patients followed at primary care facilities in Kinshasa, the capital city of Democratic Republic of the Congo.MethodsA sample of 298 hypertensive patients seen at primary healthcare facilities, 90 men and 208 women, aged ≥ 18 years, were consecutively included in this cross-sectional study. The majority (66%) was receiving monotherapy, and diuretics (43%) were the most used drugs. According to 2007 European Society of Hypertension/European Society of Cardiology hypertension guidelines, uncontrolled hypertension was defined as blood pressure ≥ 140/90 or ≥ 130/80 mmHg (diabetes or chronic kidney disease). Logistic regression analysis was used to identify the determinants of uncontrolled hypertension.ResultsUncontrolled hypertension was observed in 231 patients (77.5%), 72 men and 159 women. Uncontrolled systolic blood pressure (SBP) was more frequent than uncontrolled diastolic blood pressure (DBP) and increased significantly with advancing age (p = 0.002). The proportion of uncontrolled SBP and DBP was significantly higher in patients with renal failure (p = 0.01) and those with high (p = 0.03) to very high (p = 0.02) absolute cardiovascular risk. The metabolic syndrome (OR 2.40; 95% CI 1.01–5.74; p = 0.04) emerged as the main risk factor associated with uncontrolled hypertension.ConclusionUncontrolled hypertension was common in this case series and was associated with factors related to lifestyle and diet, which interact with blood pressure control.

Background: Hypertension is one of the major risk factors for cardiovascular diseases. Lifestyle changes, physical exercise and intake of healthy diet are some common issues associated with reducing the risk of hypertension. There are several classes of pharmacological agents used in the treatment of hypertension. Angiotensin I converting enzyme (ACE) cleaves angiotensin I to angiotensin II and inactivates bradykinin. The synthetic ACE inhibitors are used widely to treat cardiovascular disorders. They may cause many adverse effects, including dry cough, allergic reactions and skin rashes, so investigation for new natural sources could be helpful. Objectives: The aim of the present study was to evaluate angiotensin I converting enzyme inhibitory activities of hydroalcoholic extracts of Nardostachys jatamansi, Prangos ferulacea and Marrubium vulgare. Materials and Methods: ACE inhibitory activity was measured according to the methods of Cushman & Cheung with some modifications. Captopril was used as positive control. Results: The ACE inhibitory activities of hydroalcoholic extracts were as follows; M. vulgare > N. Jatamansi > P. ferulacea. The least IC 50 value was related to the hydroalcoholic extract of M. vulgare (0.791 mg/mL). The IC 50 values of N. Jatamansi and P. ferulacea were 2.147 and 4.057 mg/mL, respectively. Conclusions:The results supported the traditional antihypertensive use of these plants, especially M. vulgare, by inhibitory effects on ACE enzyme.
