Hypertension awareness, treatment and control in the community: is the ‘rule of halves’ still valid?

Abstract: One of the cornerstones of the primary prevention of hypertensive drug treatment varied between 47% and 95% in men and between 50% and 100% in women. The cardiovascular disease has been screening and early antihypertensive drug treatment of patients with high percentage of treated hypertensives achieving an adequate BP control varied between 29% and 95% in blood pressure (BP). Nevertheless, recent population studies have shown that awareness and management of men and between 0% and 100% in women. Overall, wome… Show more

“…11,17 Within the UK, data for England showed an improvement of hypertension care in the community, 10 compared to earlier studies. 18,19 A recent review of 24 studies 20 suggests that the rule of halves may no longer be valid in the industrialized world, but is probably true in developing countries. Despite this there are many people in the community with poorly controlled hypertension.…”

“…27 However, there is no doubt that the fact that one-third of the population is not aware of hypertension and, consequently, not treated, contributes to the unsatisfactory overall control of hypertension. In a review by Marques-Vidal and Tuomilehto, 28 24 population surveys of hypertension awareness, treatment, and control published since 1983 were assessed. In males, the rates for hypertension awareness among hypertensive individuals varied from 23 to 93%.…”

“…[1][2][3][4][5] Despite these known devastating complications of hypertension, blood pressure control is very poor worldwide, accounting for 34% in the United States 6 to 6% in other countries. 7 The reasons for this poor control vary greatly among different countries, but it is possible that drug selection with respect to their mechanism of action and their duration of its effect can play a significant role in the control of hypertension and the prevention of its complications. Drugs with short-to-moderate duration of action, administered in the morning, do not provide an effective 24-h blood pressure control, and especially in the following day's early morning hours when the blood pressure is the highest.…”
