Background: To compare the histopathological findings between upper and lower poles of tonsil in pediatric age group keeping in view the local and distant effects of chronic tonsillitis.Methods: Three relevant parameters, grade of inflammation (GOI), activity of inflammation and hyperplasia were histopathologically examined in the tonsils of n=60 pediatric patients with chronic tonsillitis who underwent tonsillectomy in Department of Otorhinolaryngology and Head and Neck Surgery, VIMS, Ballari, Karnataka, India. These parameters were analyzed and were subjected for statistical analysis. Results: In our study, among 60 patients, n=32 (54.2%) were female and n=28 (45.8%) were male. Those aging 5-10 years were n=34 (57.6%) and 11-15 years were n=26 (42.4%). The tonsillar enlargement of grade 3 were present in n=46 patients (77.7%), n= 12 patients (20%) had grade 2 and n=2 patients (1.3%) had grade 4.Conclusions: The GOI and hyperplasia parameters in our study have highly significant association statistically (p<0.01) in histopathology of upper poles when compared with lower poles both sides. Further, the activity of inflammation when analyzed was more in upper pole when compared with lower poles both sides, However this parameter in left side upper and lower poles of tonsil was not statistically significant . Among the 60 cases studied, in all the cases, the GOI was of high grade and activity of inflammation and hyperplasia was low grade. Chronic tonsillitis and histopathology of chronically infected tonsils becomes an important step in the management of the disease.

Introduction: Tonsillitis is an inflammation of the palatine tonsils that caused by the invasion of microorganisms into the mucous membranes of the tonsils. Age is related to the tonsils immunological activity on the size of the palatine tonsils. Oral hygiene is one of determinant risk factor for chronic tonsillitis. This study aims to provide a characteristics of age and oral hygiene in determining the size of the palatine tonsils in chronic tonsillitis patients. Methods: This systematic literature review was conducted online from January to March 2021 against articles reporting on age, palatine tonsils size, and oral hygiene on chronic tonsillitis. The search for articles referred to Preferred Reporting Items for Systematic Review and Meta-analysis (PRISMA) which conducted through PubMed, PMC, Science Direct, and Google Scholar with the with the publication year 2015-2020. Selected articles met the criteria according to the PICO framework (Population, Intervention, Comparison, Outcome). The articles were screened by checking for duplication, reading the titles and abstracts, and the full text articles. Results: Twelve in 16 research articles (75%) mentioned that there was a relation between age and enlargement of the size of the palatine tonsils in chronic tonsillitis patients, and one in 16 articles (6,25%) mentioned that there was no relation between age and tonsillar hypertrophy. Three articles (18,75%) mentioned the role and relationship of bacteria in the oral cavity related to the process of the occurrence of chronic tonsillitis. Two in 3 research articles (67%) mentioned that there was a relation between the oral hygiene status and enlargement of the size of the palatine tonsils in chronic tonsillitis patients. One in 3 articles (33%) mentioned that there was no relation. Conclusion: Ages 5-15 years had T3 tonsil size (70%), ages 13-15 years had T2 tonsil size (80%), ages 16-48 years had T2 tonsil size (76.30%) and ages 19-25 years had T3 tonsil size (53%). This means that all ages could be experience of tonsil enlargement in chronic tonsillitis. Adults had bad oral hygiene status (76.30%), meanwhile in children only 14.47% in chronic tonsillitis patients.

Background: The tonsils are lymphoepithelial tissue, it contains specialized lymphoid functional compartments which include the lymphoid follicles, parafollicular areas, crypt epithelium and high endothelial venules, which together have an essential role in the immunological process. These compartments may be altered histomorphologically throughout life time underneath common pathological condition. Aim: The aim of the current study is to evaluate special microstructural functional compartment changes as high endothelial venules, lymphoid follicles, interfollicular and connective tissue areas according to histopathological ground of the palatine tonsils. Methods: one hundred palatine tonsillar samples which were attained from patients suffering from chronic tonsillitis, recurrent tonsillitis and obstructive hypertrophic tonsils were admitted at Al-Jumhuri Teaching Hospital and Al-salaam teaching hospital in Mosul city during the period from February 2018 to February 2019. Age of patients ranged from (2-40) years. Specimens of tissue were directly fixed in 10% formalin then processed. Paraffin sections of 4μm thickness were exposed to routine stain with hematoxylin and eosin, while the studied marker (CD34) was detected by immunohistochemical method using labelled streptavidinbiotin (LSAB/HRP) method. Results: The high endothelial venules found in the subepithelial compartments as well as with in the reticulated crypt epithelium were characterized by prominent nuclei of the endothelial cells with non-epithelial cells were found on the luminal side. The mean count of high endothelial venules was peak with statistically significant in recurrent tonsillitis and hypertrophic tonsil in both surface epithelium1.67±0.24, 0.78±0.22 and crypt epithelium1.89±0.31, 0.89±0.20 (p=0.046, p= 0.032) respectively. However the percentage of follicle area compartment in the tonsillar hypertrophic cases was greater than in other infectious tonsillar diseases (03.33%) respectively. Contradictory, the interfollicular and connective tissue areas reach their maximum in chronic tonsillitis. Conclusion: The high endothelial venules are dispersed throughout the surface in addition to crypt epithelial. Follicular area percentage in the hypertrophic tonsils is higher than that in the chronic and recurrent tonsillitis group, representing a hyperplastic state of lymphoid cells in the germinal centers. It can also explain the alteration in immune defense mechanism underlying these pathological conditions.
