Hyperlipidemia and Reduced White Matter Hyperintensity Volume in Patients With Ischemic Stroke

Abstract: Background White matter hyperintensity (WMH), or leukoaraiosis, is a radiological finding generally assumed to reflect diseased small cerebral vasculature. WMH has significant functional impact through its relationship to cognitive decline and risk of ischemic and hemorrhagic stroke. Accumulating evidence suggests that some manifestations of small vessel disease such as intracerebral hemorrhage (ICH) are associated with low levels of cholesterol. We sought to determine the relationship between hyperlipidemia (… Show more

“…28 The relatively frequent use of statin may also explain the positive association between cholesterol level and log WMHv in these patients. The associations of log WMHv with hyperlipidemia and cholesterol level lost significance after an additional adjustment for statin use.…”

“…Diabetes was defined as a fasting blood glucose level ≥7.0 mmol/L or a diagnosis of diabetes requiring diet or anti-diabetic therapy. Hyperlipidemia was defined as a total cholesterol (TC) level of ≥5.72 mmol/L and/or a triglyceride (TG) level of ≥1.70 mmol/L, or treatment with a lipid-lowering medication [27]. Individuals who formerly or currently smoked ≥10 cigarettes per day for at least 2 years were defined as smokers.…”

“…Further, in 1,135 acute IS patients hypercholesterolaemia, hypertriglyceridaemia or use of lipid-lowering medication was associated with decreased WMH severity [59]. Unfortunately, the authors were unable to assess the contribution of statin therapy to the association seen, which may have confounded their findings.…”
