HyperArc VMAT planning for single and multiple brain metastases stereotactic radiosurgery: a new treatment planning approach

Abstract: PurposeThe HyperArc VMAT (HA-VMAT) planning approach was newly developed to fulfill the demands of dose delivery for brain metastases stereotactic radiosurgery. We compared the dosimetric parameters of the HA-VMAT plan with those of the conventional VMAT (C-VMAT).Material and methodsFor 23 patients (1–4 brain metastases), C-VMAT and HA-VMAT plans with a prescription dose of 20–24 Gy were retrospectively generated, and dosimetric parameters for PTV (homogeneity index, HI; conformity index, CI; gradient index, G… Show more

“…Ohira et al . demonstrated that HyperArc plans generated a significantly steeper dose falloff than that of conventional VMAT‐based SRS plans . Thus, HyperArc plans may be sensitive to rotational setup errors and worsen plan conformity easily.…”

“…The HyperArc can provide a steeper dose gradient for targets while minimizing doses to surrounding normal tissues as much as possible with a lesser workload than that of the conventional SRS technique of VMAT . In addition, single‐isocentric irradiation for multiple metastases can reduce treatment time compared with conventional multi‐isocentric irradiation treatment devices, such as the Gamma Knife and CyberKnife ® …”

“…With the recent introduction of commercial single‐isocenter planning algorithms for simultaneously treating multiple brain metastases, either using automated dynamic conformal arcs or automated VMAT, more dosimetric comparison to GK is required. These studies have already demonstrated a reduction in monitor units compared with conventional VMAT.…”

“…The TrueBeam (Varian Medical Systems) is one of the latest generations of linacs. This machine and recent technologies have enabled reduction of the treatment time by use of flattening filter‐free (FFF) beams, volumetric‐modulated arc therapy, and linac‐based single‐isocenter noncoplanar techniques . Although a few studies have investigated the inter‐unit variability of the TrueBeam, none has yet explored very small‐field (≤10 mm) dosimetry of the TrueBeam.…”
