Hydrocephalus in aqueductal stenosis

Abstract: The correct interpretation of the modern neuroradiologic techniques may help in selecting adequate treatment between the two main options (third ventriculostomy or shunting). In the last decades, endoscopic third ventriculostomy has become the first-line treatment of aqueductal stenosis; however, some issues, such as the cause of failures in well selected patients, long-term outcome in infant treated with ETV, and effect of persistent ventriculomegaly on neuropsychological developmental, remain unanswered.

“…Sylvius aqueduct stenosis is not a stable condition (1). Functional processes such as deformation of the brain stem due to the ongoing enlargement of the supratentorial ventricles may worsen the obstacle and precipitate acute intracranial hypertension.…”

“…In children and adults, aqueduct width and cross-sectional area have been measured by MRI using the midline sagittal plane, perpendicular to its long axis [14, 15,17] . The aqueductal morphology in AS included a funnel-shaped enlargement of the proximal cerebral aqueduct related to distal obstruction, a diffuse reduction in its caliber reaching complete occlusion, and the presence of an aqueductal web [2,18,19] . In our pathological cases, AS diagnosis was suspected by aqueductal occlusion in 6 cases and funnel shape in 1.…”

“…In almost 75% of patients the etiology of the disorder is not known and in the remaining can be attributed to genetic factors, infection/inflammation, hemorrhage, venous hypertension, or tumors [2] . Classically, this condition is not secondary to an underlying malformation or destructive process, so the severe ventricular dilation is accompanied by an otherwise normal brain anatomy [3,4] .…”

“…In this process a fenestration is made in the thinned floor of third ventricle which is an attempt to bypass the obstruction in the aqueduct of the Sylvius 1 . The process is not a cure for hydrocephalus rather a process for an alternative to shunt surgery 1,2 . The first open third ventriculostomy was done in 1922.…”
