a b s t r a c tBackground: Endoscopic third ventriculostomy (ETV) is the treatment of choice for obstructive hydrocephalus, but the outcome is still controversial in terms of age and aetiology. Methods: Between 1998 and 2011, 168 consecutive procedures were performed in 164 patients, primarily children (56% < 18 years of age and 35% < 2 years of age). The causes of obstructive hydrocephalus included tumoural pathology, Chiari malformation, congenital obstruction of the aqueduct, post-infectious and post-haemorrhagic membranes, and ventriculo-peritoneal shunt (VPS) malfunctions. Successful ETV was defined by the resolution of symptoms and the avoidance of a shunt. Results: ETV was successful in 75.6% of patients, but 19% of the patients required VPS in the first month after ETV, and 5.4% required a VPS more than one month after ETV. Four patients were ultimately submitted for second ETVs. In this series, no major permanent morbidity or mortality was observed. Conclusions: ETV is a safe procedure and an effective treatment for obstructive hydrocephalus even following the dysfunction of previous VPSs and in children younger than two years.

Hydrocephalus is a disorder in which an Kafkas J Med Sci 2013; 3(2):88-95 • doi: 10.5505/kjms.2013.41196 miktardaki BOS'un serebral ventriküllerde birikmesinden kaynaklanır. Daha az sıklıkta emilimdeki yetersizlik (komunike hidrosefali) BOS birikimine neden olur.Normal basınçlı hidrosefalide serebral ventriküller patolojik olarak genişlemiş fakat KİBAS normal sınırlar-dadır. Bu durum genellikle bozulmuş BOS emiliminden kaynaklanır. Hidrosefalinin bu tipi daha sıklıkla yetişkinlerde görüldüğünden burada tartışılmayacaktır.Hidrosefalinin bu formları aynı kelimeyi içeren iki radyolojik bulgudan ayırt edilmelidir; 'Hidrosefali ex-vacuo' terimi beyin atrofi sine veya yaralanmaya sekonder beyin dokusu kaybına bağlı ventriküllerde-ki dilatasyonu anlatmaktadır. Hidrosefali ex-vacuo'ya KİBAS artışı eşlik etmez. Eksternal hidrosefali terimi ise subaraknoid boşlukta fazla miktarda BOS bulunmasını ifade etmektedir 3 . EpidemiyolojiAmerika Birleşik Devletleri ve Avrupa'da konjenital hidrosefali prevalansı 0,5-0,8/1000 canlı ve ölü doğum olarak hesaplanmıştır [4][5][6] . Bu vakaların yaklaşık yarısının myelomeningosel (spina bifi da) ile ilişkili olduğu, ancak bu oranın popülasyonlar arasında değişiklik gösterdiği bildirilmektedir. FizyolojiBOS başlıca koroid pleksus tarafından yapılmak-tadır. Ventriküler sistem ve subaraknoid boşlukta dolanmakta ve buradan sistemik dolaşımca emilmektedir. BOS akımı ağırlıklı olarak kranyal bölgede gerçekleşmektedir. BOS ÜretimiKoroid pleksus BOS üretiminin %60-80'inden sorumludur. İleri derecede vaskülarize bağ dokudan GirișHidrosefali aşırı miktarda beyin omurilik sıvısının (BOS) dilate serebral ventriküller ve subaraknoid boşlukta birikmesi ile seyreden bir bozukluktur 1, 2 .Çocuklarda, hidrosefali hemen her zaman artmış kafa içi basıncı (KİBAS) ile ilişkilidir.
