Introduction Although hydatid cyst remains one of the prevalent parasitic infections in humans, hydatid cyst of the thyroid is extremely rare, even in endemic areas. Here we present two cases of thyroid hydatid cysts. Case presentation A 35 and a 50 year-old Iranian female with a positive history of animal contact were presented with a neck lump without any compressive symptoms. A physical exam revealed neck masses that elevated with swallowing. Thyroid gland ultrasonography showed cystic thyroid lesions, and fine needle aspiration (FNA) suggested a thyroid hydatic cyst. Thyroid lobectomy and isthmectomy were done for the first patient, and near-total thyroidectomy was done for the other. The pathology report confirmed the diagnosis of a hydatid cyst. None of the patients had hydatid cysts in other sites. Patients were discharged without an antiparasitic drug, and no recurrence was detected at the six-month follow-up. Conclusion It is necessary to consider hydatid cysts in the differential diagnosis of cystic lesions of the thyroid gland in endemic areas, especially in people with a positive history of animal contact.

Thyroid isthmus lesions are generally small sized and can be solid or cystic. Discerning isthmic origin of a large nodule, especially if purely cystic, can become a diagnostic challenge because of thin thyroid tissue in it. We report a case of a 68-year-old male patient who had 6 weeks history of non- inflammatory central neck swelling associated with recent dysphagia, for which he underwent ultrasound and computed tomography (CT) scan examinations. Colloid nodules usually do not require further attention. Despite being commonest and benign thyroid nodules, they may require treatment if causing pressure symptoms. Its imaging characteristics can be variable, but they usually exhibit comet tail artifacts on ultrasound. In equivocal cases, claw sign on CT scan is diagnostic to confirm the site. Radiologists have a principle role to rule out other differentials of cystic neck lesions by careful examination of imaging features. In our case, CT scan allowed to rule out primary differential of thyroglossal cyst and guided clinicians for specific management plan.

Introduction Hydatid cysts of the thyroid gland, though rare, pose a diagnostic and therapeutic challenge. This systematic review summarizes current knowledge on the presentation, diagnosis, and management. Methods A systematic review of the published studies of thyroid hydatid cysts was conducted. The included studies of thyroid hydatid cysts had the following properties: 1) The thyroid infection was confirmed by diagnostic modalities, surgical findings, or histopathology. 2) The case presentation was provided in the study. 3) The cyst(s) were originally located or adhered to the thyroid and did not rupture into it from the other adjacent structures or organs. Results Overall, 39 studies were compatible with the inclusion criteria. Turkey had the highest case occurrence (36%). Female predominance (59%) and a mean age of 38.33 years were observed. Mass (79.5%) was the most common symptom, followed by hoarseness of voice (4%). Serology (18.75% positive) and ultrasound (77.5%) were primarily used for diagnosis. Surgery was the mainstay of treatment (96%), with total thyroidectomy (32.6%) and left hemithyroidectomy (16%) being the most common approaches. No recurrence was reported in the follow-up period (0%). Conclusion This review highlights the rarity, clinical presentation, diagnosis, and successful surgical management of thyroid hydatid cysts. Further research is needed to optimize diagnostic methods and explore alternative treatment options.
