Hydatid Cyst Of Skeletal Muscle Presenting As Soft Tissue Tumour

Abstract: Hydatid disease is helminthic infection caused by tape worm echinococcus granulosis. It commonly involves liver (65-75%) and the lungs (25 to 30%). Involvement of bones and skeletal muscle is very rare, i.e. 3 to 5%. This is the report of a 21-year girl, city resident, presented with mass involving left proximal thigh. The mass was painless and had gradually increased in size over three years. MRI revealed solid cum cystic lesion in vastus medialis muscle. Tru-Cut needle biopsy of the lesion revealed necrotic … Show more

“…Hydatid cyst of muscle usually presents as a painless, slow‐growing, asymptomatic mass with a normal overlying skin, similar to our case presented above. A high index of suspicion is therefore necessary as its differential diagnosis can range from chronic hematoma and lipoma to malignant soft tissue tumors such as myxoid liposarcoma 7–9 . The diagnosis of hydatid cysts is initially sought with serology and imaging findings 10–12 .…”

“…Factors favoring infection include his immunocompromised state with a recent month-long hospital stay, his significant leukocytosis given his immunosuppression, and the progressively painful nature of his lesion. Distinction is difficult, and reports of other opportunistic infections being mistaken for solid soft-tissue tumors include hydatid cysts, Echinococcus, and cysticercosis [11][12][13][14][15] . Patients immunosuppressed after solid organ transplant are at increased risk of opportunistic infections with many pathogens that can form intramuscular abscesses, typically through either spread from cutaneous lesions or coalescence of pyomyositis [16][17][18] .…”

“…However, in non-endemic areas, ultrasound can be misleading with soft tissue tumors especially in deep locations. 16 Computed tomography (CT) radiologic features vary from a unilocular or multilocular cyst, with or without septas, debris or wall calcifications, to a complex or solid mass without enhancement on intravenous contrast. 13 The appearance of muscular hydatidosis is unoften typical and the multivesicular form is specific as is reflects multiple daughter cysts within the parent cyst.…”
