Human Usutu Virus Infection with Atypical Neurologic Presentation, Montpellier, France, 2016

Abstract: Infection with Usutu virus (USUV) has been recently associated with neurologic disorders, such as encephalitis or meningoencephalitis, in humans. These findings indicate that USUV is a potential health threat. We report an acute human infection with USUV in France putatively associated with a clinical diagnosis of idiopathic facial paralysis.

“…Due to these rare instances, and because USUV was never associated with severe disease in animals or humans, it was not perceived as a potential threat to human health (Vazquez et al, )—until 2009, when the first cases of USUV‐related encephalitis in immunocompromised patients were described in Italy, providing evidence that USUV is capable of triggering severe neurological symptoms (Cavrini et al, ; Pecorari et al, ). Moreover, in 2016, a patient in France presented with acute USUV infection and possibly associated unilateral facial paralysis (Simonin et al, ).…”

“…Considering that no concurrent wave of neuro‐invasive or other disease in humans was perceived in the Netherlands, human USUV infection so far seems rather harmless. Of course it must be noted that human disease caused by USUV infection has been reported, and that the awareness of Dutch physicians of USUV being present as a potential cause of disease is very low. Three immunocompetent recipients of USUV RNA‐positive blood components did not show signs of infection.…”

“…Since 2016 USUV causes seasonal mortality in Dutch birds. For humans, disease due to USUV infection seems limited to rare cases with non‐specific symptoms, or meningoencephalitis, or atypical neuro‐invasive disease . In Aachen, Vienna, and the Lazio region of Italy the screening of blood donors for WNV‐RNA revealed several cases of silent USUV infection …”

“…USUV infection in humans is often asymptomatic. Nonetheless, an increasing number of cases with neurological complications, such as encephalitis or meningoencephalitis, have recently been reported [3,[6][7][8][9][10][11]. In terms of animal health, USUV has been responsible for several epornitics in Europe since 1996 [12].…”
