Human Pulmonary Infection by the Zoonotic Metastrongylus salmi Nematode. The First Reported Case in the Americas

Abstract: Abstract. Pulmonary metastrongylosis, a zoonotic disease found primarily in pigs, is caused by eight different species of the cosmopolitan nematode Metastrongylus genus. To date, only four human cases have been reported, all from Europe. Herein, a severe case of pulmonary infection caused by Metastrongylus salmi in an Ecuadorian man, with successful treatment with ivermectin, is described.

“…Wild boars can harbour several Metastrongylus species, including M. asymmetricus, M. confusus , M. elongatus , M. apri , M. pudendotectus and M. salmi [ 55 57 ]. Of these, M. elongatus and M. salmi have been identified in humans as well as wild boars [ 41 , 58 ]. Metastrongylids were not identified to species level, and it is therefore unknown if zoonotic species were present.…”

“…Hence, parasitic infections may cause significant economic losses to the pig production. Additionally, the helminths A. suum and Metastrongylus are zoonotic [ 39 41 ] and the same may be true for Trichuris suis [ 35 ]. In humans, A. suum infection may cause severe visceral larval migrans causing liver and lung lesions [ 42 ].…”

“…While Metastrongylus sp. are known to occasionally infect other species including humans (Calvopina et al, 2016), the genus is generally thought to be specific to pigs. We also note that the genotype from dog 45 closely resembles that of a reptile-infecting Strongyloides sp.…”
