HRT and heart disease: problems and prospects

Abstract: The divergent findings of hormone replacement therapy (HRT) from observational and randomized clinical studies are summarized and reasons for the different results are postulated. Chronic use of HRT since menopause has no harmful effects on CHD event rate, while the initiation of therapy after a recent cardiovascular event causes an early increase in recurrent CHD events. Once endothelial dysfunction and atherosclerotic disease has developed, the starting of HRT promotes plaque instability, vascular inflammati… Show more

“…Such protective effects of twinning could be hypothesized to be brought about by higher levels of estrogens (by means of elevated follicle-stimulating hormone levels) among reproductive-age mothers who delivered twins, which may in turn reduce the risk of cardiovascular diseases later in life (reviewed in refs. 43,44). If mothers of twins turn out to have lower mortality from cardiovascular diseases also in contemporary populations, this might have medical significance, because cardiovascular diseases are a major cause of death in the developed world.…”

“…However, bisphosphonates, the most common anti-absorbent agents, can inhibit bone turnover, impair the natural mechanism of bone repair, and increase the risk of adverse complications such as jaw necrosis and atypical fractures. Furthermore, the efficacy of hormone replacement therapy decreases over time, and long-term treatment increases the risk of reproductive malignancies, cardiovascular diseases, and serious sequelae [ 5 , 6 ]. Therefore, developing a new treatment method for OP is crucial.…”

“…Estrogen exerts beneficial effects on blood lipids, lowdensity lipoprotein (LDL)/high-density lipoprotein (HDL) ratio, plasma triacylglycerol, and vascular function. [1][2][3][4] Recently, the cardiovascular benefits of hormone replacement therapy have been questioned, [5][6][7][8] and such therapy is no longer considered a viable option for reducing CVD in postmenopausal women. Therefore, other means of modulating cholesterol and reducing the risk of CVD in the absence of estrogen are desirable.…”
